El valor de proteínas de fase aguda y la prueba LightCycler ® SeptiFast en el diagnóstico de infecciones bacterianas y virales en niñosResultados. Se incluyeron 94 niños febriles. La temperatura media de la fiebre fue significativamente más alta en niños con infecciones bacterianas que con infecciones virales (p < 0,001). En 34 (72,3%) niños con infecciones bacterianas, el hemocultivo fue negativo. De ellos, 12 (35,2%) presentaron prueba SeptiFast positiva. No hubo resultados positivos en hemocultivos de niños con infecciones virales y todos tuvieron resultado negativo para la prueba SeptiFast. La media de proteína C-reactiva el primer día de hospitalización fue significativamente más alta en el grupo con infecciones bacterianas (p < 0,001) y en los días 3 y 7 junto con la procalcitonina fueron significativamente más altas en niños con infecciones bacterianas (p <0,001). La sensibilidad y especificidad de los leucocitos, la proteína C-reactiva y la procalcitonina fueron 63,8%, 44,7%, 74,5% y 78,7%, 68,1% y 100%, respectivamente. Las áreas bajo la curva de los leucocitos, la proteína C-reactiva y la procalcitonina fueron 0,519, 0,764 y 0,835, respectivamente. Conclusiones INTRODUCCIÓNLa fiebre es una de las causas más frecuentes de consulta pediátrica. 1Es difícil diferenciar entre los casos graves de infección bacteriana o viral, incluso para los médicos más experimentados. Las infecciones bacterianas son frecuentes durante la niñez y causan morbimortalidad significativa. Debido a la falta de síntomas clínicos específicos, pueden confundirse con infecciones virales y enfermedades no infecciosas. No se dispone de un indicador de laboratorio único con una alta sensibilidad y especificidad y cuyo costo sea bajo, para distinguir entre las infecciones bacterianas y virales en la etapa inicial. 2En la actualidad, el hemocultivo es el método de referencia para el diagnóstico de infecciones bacterianas. Sin embargo, el hemocultivo tiene desventajas, entre otras, que no es rápido y la sensibilidad diagnóstica e s b a j a .3,4 T a m b i é n s e r e q u i e r e tiempo para obtener los resultados del cultivo. Además, el diagnóstico temprano y el tratamiento eficaz pueden salvar vidas en los casos de infección bacteriana. Por otro lado, el uso innecesario de antibióticos cuando se sospecha una infección bacteriana grave puede prolongar la duración de la hospitalización, incrementar la resistencia bacteriana en la comunidad y aumentar el gasto incurrido por las familias y la sociedad en una enfermedad sencilla. ВВЕДЕНИЕПрактически каждый ребенок хотя бы один раз в год переносит заболевание, сопровождающееся лихорад-кой [1]. Именно этот симптом заставляет волноваться и родителей, и врачей, что приводит к назначению боль-шого числа лекарственных средств -не только жаро-понижающих, но и системных антибиотиков. Однако лечение лихорадки как таковой не должно быть само-целью: необходимо выяснить причину повышения тем-пературы и, что главное, выделить ту категорию пациен-тов, которым показано незамедлительное лечение, хотя в большинстве случаев лихорадка обусловлена вирус-ной инфекцией и не требует какого-либо лечения [1].Первичными и основными задачами врача при лихо-радке являются дифференциальная диагностика между вирусной и бактериальной инфекцией и выбор тактики ведения пациента, а уже вторичными -снижение температуры тела, особенно с учетом того, что высота лихорадки, как правило, не коррелирует со степенью тяжести заболевания [1].
