Aim. To study the proportion of coronavirus disease 2019 (COVID-19) survivors and hospitalization cases of the Russian population for the period 2020-2022, depending on socio-demographic characteristics.Material and methods. The work was performed on random samples of the population aged 35-74 years from 15 regions participating in the Epidemiology of Cardiovascular Diseases and their Risk Factors in Regions of Russian Federation-3 (ESSE-RF3) study. The examination was carried out from February 2020 to March-April 2022 (n=28718, 47% men).Results. According to a survey of sample participants, the proportion of respondents who had COVID-19 by the spring of 2022 among all surveyed was 26,0%. Among 45-54-year-old respondents, this parameter was maximum (28,0%), while among those aged 65-74-year-old — minimal (22,3%). The following categories of people were more likely to have COVID-19: women (27,9 vs 23,9% of men, p<0,001), persons with higher education (30 vs 22,6% persons without higher education, p<0,001), persons with high income (30,0 vs 24,3% low-income, p=0,002), persons with family (26,6 vs 24,8% single, p<0,001), employed persons (28,4 vs 21,7% non-employed, p<0,001), as well as those living in urban areas (26,5 vs 24,5% country people, p=0,003). But those less likely to have COVID-19 (men, people without higher education, low-income, single, unemployed or rural residents) had more severe disease course. The following categories of people had higher hospitalization rates: men (26,0 vs 20,9% women, p<0,001), patients without higher education (27,1 vs 19,6% those with higher education, p<0,001), non-employed (34,3 vs 18,4% workers), p<0,001) and rural residents (27,2 vs 22,0% of city residents, p<0,001). Hospitalization rate was not associated with the level of income and marital status. However, low-income and single people were more likely to require mechanical ventilation.Conclusion. The results obtained by survey reflect the following population data: the proportion of patients after COVID-19, hospitalization rate and the development of severe acute respiratory failure requiring mechanical ventilation among Russians for the period 2020-2022. This can complement the population data demonstrated by state statistics. Special attention from the healthcare system, both during treatment and prevention of COVID-19, requires males, people aged ≥65 years, low-income people, single people (widowers, divorced), unemployed people (pensioners, disabled people, nonemployed), rural residents and people without higher education.

The coronavirus pandemic has produced considerable effects on medical and demographic processes worldwide and in Russia in particular. The epidemic process involved a sequence of circulating SARS-CoV-2 virus strains with different mutations and this reflected in registered levels of incidence and mortality against spatial heterogeneity of socioeconomic factors in different RF regions. The aim of this study was to analyze spatial-dynamic heterogeneity of the COVID-19 epidemic process in the RF regions in 2020–2023. We performed retrospective analysis of incidence and mortality at the national and regional levels. The analysis relied on departmental statistical data provided by Rospotrebnadzor as well as public data that described the intensive indicators of the COVID-19 epidemic process and results obtained by sequencing of biomaterial samples to identify COVID-19 in them in 2020–2023. In 2020–2023 we identified five ‘waves’ of the COVID-19 epidemic processes that interchanged sequentially. Within these waves, RF regions reached local peaks in incidence with different speed. According to available data, the highest primary incidence among all the RF regions in 2021–2022 was established in Saint Petersburg (12,821.8 cases and 17,341.2 cases per 100 thousand people); the highest mortality in 2021 was detected in the Tver region (427 cases per 100 thousand people) and in the Arkhangelsk region in 2022 (350.9 cases per 100 thousand people).The greatest number of the RF regions where the incidence due to the disease was higher than its average annual level was established in October, November, December 2021 and February 2022 (51, 68, 51 and 82 RF regions accordingly). The established spatial-dynamic heterogeneity of the epidemic process may indicate that this process can be largely determined by differences in the initial socioeconomic, medical and demographic characteristics of the RF religions. Limitations of the study are related to the used statistical data on registered incidence and mortality as well as the concept of the epidemiological ‘wave’ accepted in it. The identified territorial differences in the COVID-19 epidemic process should be considered when developing optimal regulatory impacts including those aimed at predicting probable emergent infections.

Aims: to study the epidemic process, clinical and epidemiological features of adult patients with COVID-19 identified during the first and second periods of the rise and decline in the incidence of new coronavirus infection in Russia. Materials and methods: this study included patients aged ≥18 years with a confirmed diagnosis of COVID-19, identified in the periods from 03/02/2020 to 30/06/2020 (n=286 205) and from 11/01/2020 to 01/31/2021 (n=1 655 022) in Russia. Results: at the early stage of the COVID-19 epidemic in Russia, there were defined 2 periods of rise and fall in incidence: March - August 2020 and September 2020 - May 2021. The median age of patients with COVID-19 in the first period was 50.0 [37- 62] years, in the second – 52.0 [39-64] years. The share of females was 55.5% and 60.1%, respectively. The distribution of patients in the first and second periods was: mild - 63.0% and 74.4%, moderate - 29.0% and 20.1%, severe - 4.9% and 3.5%, extremely severe – 3.1% and 2.1%, respectively. In the first period, the structure of cases was dominated by people in the age group 50-59 years (20.5%), in the second - 60-69 years old (20.5%). The median duration of the period from the onset of symptoms to diagnosis was 4 days in both periods, of the disease was 16.0 [12-21] and 13 [10-17], of hospitalization was 15.0 [12-20] and 13.0 [10-18]. The rate of hospitalizations was 48.4% and 25.6% in the first and second periods, rate of transfers to the ICU - 7.8% and 10.3%, IMV - 5.6% and 7.7%, respectively. In both periods, the median age of the deceased was 73 [66-82] years, with a higher proportion of men in all age groups except 18-29 and 70+ years. One or more chronic diseases, male gender increased the likelihood of death (OR=10.2 and OR=1.3 in the first period; OR=16.0 and OR=1.6 in the second, respectively). Conclusions: in the early period of the COVID-19 epidemic in Russia, related to the spread of the “wild” strain of SARS-COV-2 and genetically closely related lines, differences in the manifestations of the epidemic process and clinical and epidemiological characteristics of patients were revealed. In the second period there was a decrease in the frequency of the heavy and extremely severe COVID-19, a decrease in the frequency and duration of hospitalizations, but a slight increase in the frequency of ICU and IMV was noted.
