Comparison of the Effect of Three Suturing Techniques on Postkeratoplasty Astigmatism in Keratoconus

Abstract: Postkeratoplasty astigmatism and BCVA are comparable with the 3 common suturing techniques (IR, SR, and CIR) in patients with keratoconus, provided that regular postoperative examinations and topography-guided suture adjustment and/or removal are performed.

“…33,34 It is affected by surgical/suturing techniques, type/size of trephine, and proper positioning of graft and host. 35,36 The original KC host tissue may continue to exhibit KC disease progression, so it is possible that the thickening of the epithelium at the GHJ found in a significant proportion of the group 1/3 buttons may be because of increased physical stress at the graft margin during wound healing between the weakened KC host and the normal-thickness graft.…”

“…However, Karabatsas et al 35 report no significant difference in the post-keratoplasty astigmatism between the adjustable single continuous suture and combined continuous and interrupted sutures at 1 year. Recently, Javadi et al 36 have shown that postkeratoplasty astigmatism is fairly comparable with the 3 common suturing techniques. Intraoperative suture adjustment increased refractive stability and minimized final astigmatism when all sutures were removed compared with postoperative suture adjustment.…”

“…Poder-se-ia argumentar que a técnica de sutura empregada, com pontos separados, poderia ter contribuído também para os resultados. No entanto Javadi et al (18) , comprovaram que a sutura contínua, interrompida, dupla ou a combinação da sutura contínua e interrompida são comparáveis quanto ao astigmatismo resultante após 12 meses da cirurgia e dois meses após a retirada completa das suturas.…”
