Caso ClíniCo RESUMEN La gingivitis descamativa asociada con penfigoide de membranas mucosas (MMP) (ICD-10 L10), es una entidad correspondiente a enfermedad gingival no relacionada con placa bacteriana, que forma parte de un grupo heterogéneo de desórdenes ampollares, autoinmunes y crónicos, que se manifiestan clínicamente como áreas con eritema sintomáticas sobre la encía, las cuales forman una bulla ante el frotamiento, lo que se conoce como Nikolsky positivo. El estudio histopatológico se caracteriza por la división sub-basal, con presencia de infiltrado inflamatorio en la lámina propia pertinente a los depósitos de proteínas del complemento e inmunoglobulinas. El examen de inmunofluorescencia directa muestra depósitos lineales de inmunoglobulinas, principalmente IgG y complemento sobre la membrana basal. Se describe un caso donde se examinó la relación entre el periodoncista y el patólogo bucal, para el diagnóstico de penfigoide de membranas mucosas.

Purpose: With the popularity of implant therapy, clinicians need to know about treating the dental implant in patients with gingival involvement of oral lichen planus (OLP). The aim of this study is to evaluate the survival and success rates of dental implant and propose of clinical guidelines for implant treatment in OLP patient with gingival involvement. Methods: A literature search was performed in PubMed/Medline, and Cochrane database. Papers in English language published between 1990 and 2019 were evaluated. The focused questions were following; 1) Dose gingival OLP affect the survival and success rates of dental implants? 2) The management of OLP patients with gingival involvement receiving dental implant. Results: There was no study about the evaluation of dental implant only in gingival OLP patient. Five studies evaluating dental implants in OLP patients were included in this review. Implant survival rate was 100.0% in well-controlled OLP patients in all included studies. The use of topical/systemic corticosteroid in OLP patients was performed before and/or after implant placement in all included studies. Conclusions: The implant survival and success rates in well-controlled OLP patients did not different from that of non-OLP healthy subjects. The gingival OLP is associated with higher rate of peri-implant mucositis. Adequate management of gingival OLP lesions before and after implant insertion is required to reduce inflammation and associated bone loss.

Dental implants in patients with oral autoimmune diseases

Oral mucosal autoimmune diseases include a variety of disorders, like oral lichen planus (OLP), pemphigus vulgaris (PV), mucous membrane (MMP) and bullous pemphigoid (BP), epidermolysis bullosa acquisita (EBA), systemic lupus erythematosus (SLE) and Sjögren syndrome (SS) and can cause painful erosions, blisters and ulceration at the oral epithelium. Such diseases complicate dental hygiene and can lead to tooth loss. When natural teeth are missing, dental implants can improve quality of life for these patients. Osseointegration is necessary for dental implant success and is not contraindicated for patients with oral mucosal autoimmune diseases. However, the clinical dentist must consider the possible impact of oral autoimmune diseases on the oral epithelium that might affect implant success, also due to the difficulty of everyday oral hygiene, leading to bone absorption around the implant. Moreover, medication approved for the therapy of such diseases, such as corticosteroids, immunosuppressants and non-steroidal anti-inflammatory drugs (NSAIDs) could provoke osseointegration, as it compromises bone quality and affects the patient's general health. However, the impact of these drugs on implant surgery depends on dose and duration of the drug and usually dental implantation is possible.
