Objectives: The purpose of this study was to compare the surgical outcomes of Polycel® and titanium in ossiculoplasty following tympanomastoidectomy (TM). Methods: A total of 221 patients underwent ossiculoplasty following TM by a single surgeon using either Polycel® or titanium as prosthesis. Hearing was tested preoperatively and postoperatively at 6 months by pure-tone audiometry. Successful surgery was defined if postoperative air-bone gap (ABG) was <20 dB, the gain in air conduction (AC) hearing was >15 dB HL, or postoperative AC was <30 dB HL. Multiple linear regression was conducted to identify the factors associated with the surgical outcomes. Results: In canal wall up mastoidectomy (CWUM), both Polycel® and titanium showed favorable successful rates if partial ossicular replacement prosthesis (PORP) was used (64.3% of Polycel® and 67.6% in titanium). If total ossicular replacement prosthesis (TORP) was used, both represented similar outcomes (54.5% of Polycel® and 75.0% in titanium). In canal wall down mastoidectomy (CWDM), significant ABG reductions were observed only in the titanium group (5.2 ± 14.7 dB of Polycel® [ P = .083] and 7.0 ± 14.2 dB of titanium [ P = .002] in PORP; 4.6 ± 13.5 dB of Polycel® [ P = .097] and 9.5 ± 11.2 dB of titanium [ P < .001] in TORP). In multivariate analysis, titanium had a positive effect on the reduction of postoperative AC thresholds (B: −4.772; 95% CI: −8.706-−0.838). Conclusions: Both Polycel® and titanium showed favorable surgical outcomes for ossiculoplasty following CWUM. Titanium prosthesis is recommended for surgery after CWDM.

Background We explored a new method for measuring the length of artificial stapes during stapedotomy in otoendoscopic middle-ear surgery using a flexible ruler. Material/Methods A retrospective analysis was conducted on 56 cases of otosclerosis, comprising 56 ears with a follow-up of over 6 months and complete data. Patients were admitted to the Department of Otology, Huazhong University of Science and Technology Union Shenzhen Hospital from July 2020 to June 2022. SPSS statistical software was used for efficacy analysis. The results of different measurement methods on the implantation time of the prosthesis and postoperative hearing follow-up were compared. Results The 56 patients were randomly divided into 2 groups. In 1 case measured by the metal measuring stick, the prosthesis was too short and was replaced during the operation; 2 patients experienced transient dizziness postoperatively, and the other patients had no surgical complications. All 56 patients had varying degrees of hearing improvement after surgery, and no dislocation of the ossicular chain was observed during the follow-up. There was a statistically significant difference in the implantation time of the prosthesis between the different measurement methods ( P <0.05). The improvement in hearing in the flexible ruler group was statistically significant ( P <0.05). Conclusions The selection of the length of the ossicular prosthesis is a crucial step in the operation and can directly affect hearing outcomes. The use of a flexible ruler to measure prosthesis length can significantly shorten operation time and improve hearing, and the ruler is easily obtainable. It is worth promoting its application in ear endoscopic stapedotomy.

Titanium-Biohybrid Middle Ear Prostheses: A Preliminary In Vitro Study

Tissue bioengineering has been assuming a pivotal role in implementing alternatives to conven-tional ossicular middle ear replacement prostheses to overcome extrusion while preserving acoustic properties. This in vitro study aims to explore for the first time in current literature the feasibility of a biohybrid middle ear prosthesis composed of titanium surrounded by bone ex-tracellular matrix as bio-coating. We have hereby studied the adhesion and proliferation of human adipose-derived mesenchymal stem cells (hASC) on titanium scaffolds in vitro. Moreover, we identified the osteogenic differentiation of hASC using immunofluorescence assay to analyze osteoblasts gene expression profile (Alp, Runx2, Col1a1, Osx, and Bglap) and we accounted the presence of collagen as a marker of hASC ability to secrete extracellular matrix. We utilized scanning electron microscopy to evaluate the presence of extracellular matrix on the scaffolds. Our preliminary data demonstrated the titanium ability to support human adipose-derived mesenchymal stem cell colonization, proliferation, and osteoblastic differentiation, to obtain a biohybrid device. Our experience seems encouraging, we advocate further in vivo research to corroborate our results towards bone transplantation.
