BackgroundWound healing is a natural process that enables tissue repair after an injury. To shorten its duration and minimize associated complications, wounds are treated with medications. Currently there is a growing interest in the use of alternative wound dressing agents such as plant extracts. One plant used traditionally in wound treatment is Pupalia lappacea. In view of its use in wound care, we investigated the wound healing activities of 80% methanolic leave extract of Pupalia lappacea using excision, incision and dead space wound models. Also its effects on three common wound contaminants were investigated.MethodsExcision wounds were created, contaminated with microbes and treated with ointments (10% and 20% w/w) prepared from Pupalia lappacea. Incision and dead space wounds were also created in rats which were subsequently dosed orally with the extract. The wound healing activities of Pupalia lappacea ointment on excision wound was assessed by rates of wound contraction and epithelialization as well as its antibacterial effects. The effects of Pupalia lappacea on incision and dead-space wounds were determined by the wound breaking strengths and weights of the granuloma tissues formed, respectively.ResultsPupalia. lappacea ointments significantly (p < 0.05) accelerated wound healing with 20% ointment having the highest percentage wound contraction and rate of epithelialization. At 4, 7 and 14 days post treatment, mean total viable bacterial count of excision wounds of the extract treated groups were significantly (p < 0.05) lower compared against the control. Wound breaking strengths and weights of granuloma tissues formed in the extract treated groups were significantly (p < 0.05) higher than those of the control group. The minimum inhibitory concentration values obtained for the Pupalia lappacea extract against Pseudomonas aeruginosa, Staphylococcus aureus and Bacillus subtilis were 9 mg/ml, 4 mg/ml and 3 mg/ml, respectively, while the corresponding minimum bactericidal concentrations were 10 mg/ml, 8 mg/ml and 7 mg/ml.ConclusionThe results obtained showed that Pupalia. lappacea has good wound healing and antibacterial activities. These findings validate the use of this plant in traditional medicine for treatment of wounds.

Human herpesviruses can be found worldwide and cause many viral infections in immunocompetent as well as in immunocompromised patients. Herpes simplex virus (HSV) diseases can be the cause of life-threatening disease, especially in neonates. After initial infection, HSV persists latently in host neurons with the risk of periodical reactivation over a lifetime. The development of acyclovir, a potent and specific nucleoside inhibitor of the herpes DNA polymerase, was a milestone in the history of antiviral drugs in the late 1970s. During the last decades a better understanding of the replication and disease-causing state of HSV types 1 and 2 has been achieved enabling the development of new and potent antiviral compounds. In the mid-1990s, for example, valacyclovir and famciclovir were launched as prodrugs of acyclovir with improved bioavailability. Despite the numerous drugs available for the systemic treatment of HSV infections, the topical application of a cream containing an antiviral agent is still the most convenient method of treating herpes simplex labialis/facialis in the general population. For some time, the topical standard treatment for recurrent HSV infections has been acyclovir cream, despite the fact that the evidence for efficacy in recurrent episodes has been equivocal. Penciclovir, a novel acyclic nucleoside analogue, has demonstrated efficacy against HSV types 1 and 2 and seems to have a pharmacological advantage due to a prolonged half-life of its active form in HSV-infected cells. This review discusses and compares the topical treatment modalities available for HSV infections. As a conclusion, different studies are available that have shown that it is possible to reduce viral replication and hasten lesion resolution with 1% penciclovir treatment beyond the prodromal phase of the HSV infection. Comparing data of topical treatment with acyclovir and penciclovir revealed a superiority for penciclovir cream showing a significant decrease in time to lesion healing, lesion area and pain. While systemic acyclovir or valacyclovir may be valid drugs especially for HSV prophylaxis, 1% penciclovir cream should be preferred as topical treatment since there are good therapeutic results independent of the phase of development of herpetic eruptions.

As has been shown previously, phosphite of acycloguanosine (Hp-ACG) exhibits equal efficacy against ACV-sensitive and ACV-resistant HSV-1 strains in cell culture. Intraperitoneal administration of Hp-ACG to model mice with herpetic encephalitis caused by HSV-1 infection was shown to be effective in protecting against death. In the present work, we continue the study of the antiviral efficiency of Hp-ACG against HSV administered non-invasively; namely in vivo, orally and in the form of ointment formulations. It has been first shown that oral administration of Hp-ACG twice daily for five days prevents systemic infection in mice caused by HSV-1. Mortality in the control group of animals was 57%. Administration of Hp-ACG at doses of 600, 800 and 1,000 mg/kg per day significantly increased the survival and median day of death of the animals compared to the placebo-treated control group. A comparative evaluation of the therapeutic efficacy parameters of polyethylene glycol-based ACV ointment and Hp-ACG ointment was carried out after a 5-day course in the model of an experimental cutaneous infection of HSV-1 in guinea pigs. It was found that Hp-ACG has a significant therapeutic effect resulting in a statistically significant reduction in the lesion’s surface area and the amount of vesicular structures. The exhibited therapeutic effect of 10% Hp-ACG in ointment form compares well with that of 5% ACG ointment.
