Introduction: Monocyte to high-density lipoprotein (HDL) cholesterol ratio (MHR) has emerged as a novel marker of endothelial injury, inflammation, and oxidative stress. This study aimed to investigate the effect of MHR on primary nephrotic syndrome (NS) and its relationship with the severity of proteinuria.Methods: This study enrolled 161 patients newly diagnosed with primary NS and 100 healthy individuals. Demographic characteristics of the patients, primary NS diagnosis, basal laboratory parameters, the amount of 24-hour urinary protein excretion, and MHR were recorded. The groups were compared regarding these parameters.Results: MHR was significantly higher in patients with primary NS compared with the healthy group (12.89 ± 4.86 and 9.71 ± 2.30, respectively; p < 0.001). There was no difference between the groups in terms of age and sex. The amount of protein in the 24-hour urine in patients with a diagnosis of primary NS was 6.91 ± 3.73 g/day. The correlation analysis showed a positive correlation between MHR and the amount of proteinuria (r = 0.519, p < 0.001) and creatinine level (r = 0.167, p = 0.034). The multivariate regression analysis found that the severity of proteinuria was independently correlated to MHR (p < 0.001). According to the receiver operating characteristic curve analysis, the optimal cut-off level for MHR in NS was 10.08 (area under the curve of 0.704, sensitivity of 68%, and a specificity of 62%). Conclusion:Our study is the first to compare the severity of proteinuria and renal functions with MHR in patients with primary NS. We believe that MHR can be used as a biomarker to determine inflammation, endothelial injury, and the level of oxidative stress, and may be useful to predict prognosis in patients with primary NS.

Objective: We purposed to evaluate the association between Monocyte To High-Density Lipoprotein Cholesterol Ratio (MHR) and mortality during a year follow-up in patients with reduced efection fraction heart failure (HFrEF). Method:We included total of 552 patients with HFrEF into the study. The main outcome was mortality at 1-year followup. Sociodemographic characteristics, hemogram and biochemical parameters, ejection fractions of patients were recorded. MHR was was computed by splitting monocyte into high-density lipoprotein cholesterol (HDL-C). Results:The HFrEF population were seperated into different groups as non-survivors (n=45) and survivors (n=507). When the groups are compared the non-survivors group was older (p=0.04) and chronic renal failure incidence was higher in non-survivors (p=0.001). The MHR was notably higher in the non-survivors group (p=0.000). We have shown that MHR had a positive correlation with mortality (r=0.146 p=0.001). Also higher MHR was found as a strong paramer to predict of mortality (Odds ratio: 2.471, 95% confidence interval: 1.729-4.092, P=0.005). The cut-off values of 6,39 might be applied for predicting the mortality with 88% sensitivity and 97% specificity [Area under curve:0.831 (95% Confidence Interval: 0.799-0.869) p=0.001]. Conclusion:MHR can be used as an easy and usefull tool to predict mortality in HFrEF patients.

In this study, we aimed to evaluate the effect of monocyte/high-density lipoprotein (MHR), which is associated with systemic inflammation: on prolonged hospitalization in patients with mild acute pancreatitis. Patients hospitalized for acute edematous pancreatitis between 01.01.2021 and 31.12.2021 were retrospectively screened. Arrival Ranson scores of the patients were calculated. Those with a Ranson score <3 were considered as mild acute pancreatitis and were included in the study. Patients with mild acute pancreatitis were divided into 2 groups with a hospital stay of <8 days and ≥8 days. Monocyte/HDL, biochemical and metabolic parameters were compared between the groups. The study was conducted with a total of 39 patients, 23 male (59%) and 16 female (41%). While 28 (71.8%) of the patients were discharged within <8 days, 11 (28.2%) were hospitalized for ≥8 days. Group ≥8 days was considered as prolonged hospitalization. It was determined that the monocyte/HDL ratio was significantly higher in the group with prolonged hospitalization between the groups. In univariate analyzes, it was determined that the presence of Diabetes Mellitus and MHR increased the risk of prolonged hospitalization by 5.25 times and 1.085 times, respectively. In the multivariate analysis of these two parameters, MHR was found to be an independent risk factor for prolonged hospitalization. It was concluded that monocyte/HDL can be used as a simple and reliable parameter to predict the duration of hospitalization in patients with mild acute pancreatitis.
