Objective: To evaluate aberrometric changes in corneas with keratoconus implanted with intrastromal corneal ring segment (Keraring 355) during a 3-months follow-up. Design: Prospective, consecutive case seriesMethods: In this interventional study, 22 eyes of 21 keratoconic patients with mean age of 29 ± 6 years who underwent KeraRing 355 intrastromal corneal ring insertion using a Pocket Maker Microkeratome for channel creation and completed at least three months of follow-up were included. Uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), refractive outcome, Orbscan finding and corneal aberrations were assessed.Results: Three months postoperately, Low order aberration (defocus and astigmatism) decreased significantly (P<0.001). No statistically significant changes were detected in total RMS, trefoil, secondary trefoil and coma. Moreover, a statistically significant increase in quatrefoil, secondary coma, spherical aberration and secondary spherical was found. In term of visual outcomes, the mean UCVA (in LogMAR value) improved significantly from 0.75 ± 0.33 to 0.31 ± 0.23 and the mean BSCVA improved (from 0.31 ± 0.16 to 0.19 ± 0.15) (P<0.05). The mean spherical refractive error improved (from -1.37 ± 1.36 to 0.01 ± 2.47) and the mean cylindrical refractive error decreased significantly (from -3.8 ± 1.03 to -2.1 ± 1.27 (P<0.001)). Also, the mean keratometry value decreased from 47.61 ± 2.03 to 42.50 ± 2.76. Conclusion: KeraRing 355 implantation provided significant improvement in low order aberrations but was not effective in high order aberrations (HOA) in low/moderate keratoconus patients. However, UCVA, BCVA, spherical and cylindrical refractive error and keratometry results improved significantly.

Purpose To analyze whether an association exists between keratometric and pachymetric changes in the cornea, and whether it can be used to create pachymetric cutoff criteria secondary to keratometric criteria. Methods In this cross-sectional study, 1000 candidates presenting to the refractive surgery services of a tertiary care hospital underwent bilateral Orbscan IIz (Bausch and Lomb) assessment along with other ophthalmic evaluation.Results Stepwise regression analysis-based models showed that simulated keratometry (simK) astigmatism was significantly predicted by the minimum corneal thickness (MCT) and difference between central and MCT (dCT), mean SimK by the MCT and dCT, and maximum keratometry in the central 10-mm zone by the MCT and dCT (Po0.001). The mean MCT values were 542.5±39.6, 539.9±39.2, 524.2±49.5, and 449.3±73.7 lm for flatter normal (o44 D), steeper normal (Z44 D), keratoconus suspect and keratoconic eyes, respectively (Po0.001). The mean differences between central corneal thickness and MCT (dCT) were 12.2 ± 7.1 lm, 12.4 ± 7.4 lm, 14.4 ± 8.9 lm and 23.2 ± 10.1 lm for the flatter normal, steeper normal, keratoconus suspect, and keratoconic eyes, respectively (Po0.001). Mean and 2SD cutoff were used to suggest that a cornea having MCTo461 lm or dCT427 lm has only a 2.5% chance of being normal and not a keratoconus suspect or worse. Conclusion Pachymetric diagnostic cutoffs can be used as adjuncts to the existing topographic criteria to screen keratoconus suspect and keratoconic eyes.

To report intraoperative and 24-month refractive, topographic, tomographic, and aberrometric outcomes after corneal collagen cross-linking in progressive advanced keratoconus. Methods: Prospective, nonrandomized single-center clinical study involving 28 eyes. Main outcome measures included uncorrected and best spectaclecorrected visual acuities, sphere and cylinder refraction, topography, tomography, aberrometry, and endothelial cell count evaluated at baseline and follow-up at 1, 3, 6, 12, and 24 months after treatment. Topography was also recorded intraoperatively.
