Comparison of electro and chemical cautery in the treatment of anterior epistaxis

Abstract: In the ENT Department of the Royal Victoria Hospital, Belfast, the impression (supported only by anecdotes) was that electro-cautery was superior to chemical cautery in the treatment of simple anterior epistaxis. Since no evaluation of the relative merits of electro and chemical cautery has been reported, a prospective randomized study was conducted to assess the effectiveness of electro-cautery and cautery with silver nitrate. The results of the study showed that there was no statistically significant differe… Show more

“…The superiority of one surgical treatment versus another surgical treatment has yet to be demonstrated in either an observational study or a randomized clinical trial. Reported failure rates for arterial ligation ( 1, 2 ), endoscopic cautery ( 3‐5 ), and angiographic embolization ( 6‐9 ) each range from approximately 10 to 25%. The reported complication rates for arterial ligation and embolization are also very similar ( 1, 6, 7 ).…”

“…11,12 The solid form of silver nitrate is used for chemical cauterization and antisepsis in the treatment of epistaxis and neonatal umbilical granulomas, whereas the solution form is widely used for prophylaxis of neonatal conjunctivitis. [13][14][15][16][17] The solution form of silver nitrate is also used for filarial chyluria in urology. 18,19 In addition, the solid form of silver nitrate has been found to be successful in the treatment of wart and nail granulomas and oral aphthous stomatitis.…”

“…Oxymetazoline and anterior pressure is the first step in management of anterior bleeds, which has been found to control 65% of nosebleeds presenting to the emergency department . If topical sprays fail to control bleeding, visualization with a headlight or endoscopes to identify the focus of bleeding should be attempted—followed by cautery with silver nitrate or electrocautery, which have been found to be equally effective with the same rate of complications . If the above interventions fail, anterior nasal packing can be placed, with the purpose of controlling bleeding while minimizing additional mucosal trauma .…”
