Comparison of CT and MRI in patients with tibial plateau fracture: can CT findings predict ligament tear or meniscal injury?

Abstract: In the acute setting, CT offers high sensitivity and specificity for depicting osseous avulsions, as well as high negative predictive value for excluding ligament injury. However, MRI remains necessary for the preoperative detection of meniscal injury.

“…External circular fixation is used for complex fractures with soft-tissue damage because of the advantage of being minimally invasive and a potential to reduce deep infection [8, 15–23]. External fixators are also used as a temporary stabilization frames across the knee joint for pain relief, provisional reduction and soft-tissue control; CT scans are obtained for pre-operative planning [17, 24, 25]. Recently, we have observed a progressive modification of the treatment from ORIF to arthroscopic assisted reduction and internal fixation (ARIF) [14, 24, 2628].…”

Recently, we have observed a progressive modification of the treatment from ORIF to arthroscopic assisted reduction and internal fixation (ARIF) [14, 24, 2628]. Some authors recommend ARIF for Schatzker type I, II, III, IV whilst a few have done the same for type V and VI fractures [6, 9, 14, 23, 2831].…”

“…Although no MR images were obtained in this case, MRI can be used to look for meniscal injury ( 4 ). When MR imaging is compared to arthroscopy, MRI has a higher negative predictive value of 92.8% for meniscal and ligament injury than arthroscopy, but it only has an 85% accuracy in diagnosis of meniscal and ACL tears ( 2 ).…”

“…CT was found to be sensitive and specific in identifying ligament injuries because most of them had at least small bony avulsions, but MRI was necessary to detect meniscal injuries [22]. CT-angiography must be performed in cases where a vessel damage is suspected.…”
