Comparison of corneal keratocytes before and after corneal collagen cross-linking in keratoconus patients

Abstract: Keratoconus is a bilateral and asymmetric corneal ectasia. Cross-linking uses ultra-violate rays to enhance corneal tissue. The purpose of this study was to use in vivo confocal microscopy to quantitatively analyze microstructural changes over time, after corneal collagen cross-linking (CXL) in keratoconus patients. In this before-and-after study, a total of 45 keratoconic eyes were selected for cross-linking among patients referred to Al-Zahra ophthalmology clinic during 2013-2014. All patients underwent comp… Show more

“…Furthermore, the IVCM revealed keratocyte apoptosis seen by lower keratocyte cell count in CXL treated inlays than in untreated inlays that is consistent with previous studies of CXL in human corneas 36,37 . Increased keratocyte intensity was observed following CXL, likely due to the presence of activated keratocytes 37 that may contribute to the CXL-associated corneal haze 38 . The postoperative regime included topical 0.3% tobramycin and 0.1% dexamethasone four times daily for 1 week in all groups.…”

“…In the present study, confocal microscopic changes after MyoRing implantation showed some similarities with the changes after corneal collagen cross-linking. 13 , 14 , 15 After both procedures, early loss of subepithelial nerve plexus, subsequent regeneration of nerve plexus, and early decrease in stromal keratocyte density were seen. 13 , 14 , 15 These two procedures halt the progression of keratoconus with different mechanisms, but the result on cornea may be similar.…”

“…In groups A and B, many cells with thick and visible processes, and oval, brightly reflective keratocyte nuclei were seen. These cells were assumed to be activated keratocytes [ 12 ]. Many activated keratocytes were found to occupy the peripheral lamellar interface and beneath the flap edge, aggregating into a dense cluster mainly in the areas where the previous nylon sutures and fibrin glue were placed.…”
