Comparison of clinical outcomes following treatment of chronic adult periodontitis with subgingival scaling or subgingival scaling plus metronidazole gel

Abstract: At the end of the study, the mean reductions for PPD were 1.0 mm (SRP) compared to 1.5 mm (SRP+gel), and for CAL they were 0.4 mm (SRP) compared to 0.8 mm (SRP+gel), with mean difference for CAL between treatments of 0.4 +/- 0.6 mm (95% confidence intervals of 0.3-0.6 mm). The combination therapy of SRP+gel was superior to the conventional treatment of SRP alone, and these differences were maintained for 9 months.

“…The results are superior to commercially available products (e.g., ELYZOL®, a MET containing gel) that need to be applied with greater drug concentrations over a 7-day period [39,40]. Finally, for the antibacterial experiments we chose the periodontal pathogen Porphyromonas gingivalis [42], one of the major virulent pathogens associated with periodontal disease and some device-associated infections such as peri-implantitis [43,44].…”

“…As an example, a commercially available gel may be cited containing 25% of metronidazole, which could release the drug for no more than 36 hours. 16 While some studies with this gel demonstrated improvements in clinical and microbiological parameters of periodontitis after subgingival application as a monotherapy 24 or when associated with conventional mechanical therapy, [25][26][27] others did not observe such advantages in the use of this association. [28][29][30] Studies to assess the effects of this gel on clinical and microbiological parameters of periodontitis are currently being conducted and the results will complement the evaluation of this thermoreversible gel as a local delivery system for periodontitis treatment.…”

“…Regarding adjunctive local (SRPloc) therapy, irrigation of the pockets with disinfectants, application of local drug delivering devices [17][18][19][20][21][22] or local laser treatment [23][24][25][26] were identified. Pocket irrigation and mouth rinses in most of the studies were performed with chlorhexidine [27][28][29]66].…”
