Patients using fixed orthodontic appliance are more susceptible to calculus formation than patient without orthodontic treatment. Pyrophosphate and papain toothpaste can be used to reduce dental plaque and calculus accumulation. The aim of this study was to analyze the differences in the calcium level and phosphate level in saliva, also saliva pH in patient's after treated with toothpaste containing pyrophosphate and papain. This study was true experimental study with pre-test and post-test controls group design with random sampling. Sample size was (n=20). Sample was 17-22 years old male or female patient using fixed orthodontic appliance. Sample divided into two groups: control group treated with placebo toothpaste and treatment group treated with toothpaste containing pyrophosphate and papain with blind method. Patients were instructed to brush their tooth twice a day daily. 3 ml unstimulated saliva was collected on the Day 1, 3, and 5. Spectrophotometer Atom Absorbance (SAA) was used to measure calcium level and phosphate level in saliva, while saliva pH measurement using pH meters. Treatment group showed lowest mean (SD) calcium level [1.12(0.29]. Calcium level in treatment group between groups (p<0.05) was significantly different. Saliva pH was insignificantly different between groups (p>0.05). Treatment group showed lowest phosphate level in Day 5. Phosphate level in treatment group between groups (p<0.05) was significantly different. Toothpastes containing pyrophosphate and papain decreased calcium levels, phosphate levels effectively and maintaining normal salivary pH to inhibit calculus formation in patients using fixed orthodontic appliance.

Background: Antibacterial agents have always been the part and parcel in the treatment of periodontal infection. One such known agent is Active Leptospermum Honey (ALH) which has been the most studied species of medical grade honey used for the treatment of wounds and burns. Therefore, this study revolves around evaluating the efficacy of active leptospermum honey as a local drug delivery (LDD) agent on probing pocket depth (PPD), relative attachment level (RAL), and periodontal pocket pH. Materials and Methods: Based on the study's inclusion and exclusion criteria, a total of 100 sites with probing pocket depth of ≥ 5mm in posterior region will be selected. The sites will be grouped as: Test Group: 50 sites, Control Group: 50 sites, following the assessment of baseline clinical and biochemical parameters. At the baseline appointment, each patient received full mouth scaling and root planning (SRP). They were called back after 48 hours. Following the assessment of biochemical parameters by using pH test paper strips, MEDIHONEY® was placed as a local drug delivery (LDD) agent in test sites. In control sites only SRP was done. Patients were recalled after 30 days following LDD to reassess only gingival index, plaque index and biochemical parameters. Patients were recalled at 90 days following LDD for the reassessment of all clinical and biochemical parameters. Results: Plaque and gingival inflammation reduced in both groups but test group showed more reduction. Pocket depths were reduced significantly in test site. There was significant pH reduction in test sites. Clinical attachment level was also seen to be improved in the sites which were tested. Conclusion: When administered locally, Active Leptospermum Honey demonstrated lessening of PPD, gingival index (GI), and plaque index (PI), as well as higher levels of clinical attachment (CA).

Periodontal disease is oral health problem which attacks 20-50% of the global population. In Indonesia, 60% population suffers periodontal disease and become the second most common disease in the oral cavity. Aggressive periodontitis (AP) is one of periodontal disease, mainly attacks individual under 30 years-old, with rapid alveolar bone destruction. Bone graft implantation surgery is the current treatment for AP, but it is invasive and high risk infection. Nanohydroxyapatite-chitosan (nHPA-CS) hydrogel injectable scaffold is non-invasive treatment that contains bone mineral hydroxyapatite that derived from bovine bone and chitosan (CS) biopolymer with low immunogenicity. Platelet-Rich-Fibrin (PRF) contains huge amount growth factor which is biodegradable to human body. Buccal-Fat-Pad Stem-Cell (BFPSC) was chosen because it is easy to harvest and able to differentiate into mesenchymal lineage tissue. The aim of this narrative review is to describe the potential combination of nanohydroxyapatite-chitosan hydrogel scaffold, platelet-rich-fibrin, and buccal-fat-pad stem-cells for regenerating alveolar bone in aggressive-periodontitis patients. Data was obtained by literature study scientific research articles. In patients with AP, BFPSC can differentiate into osteoblast to improve osteogenesis, nHPA-CS scaffold creates host like tissue microenvironment to stimulate osteoinduction and osteoconduction activity of BFPSC, while PRF stimulates osteogenic differentiation and proliferation of BFPSC with insulin growth-factor, platelets derived growth-factor, transforming growth-factor, fibroblast growth-factor, vascular endothelial growth-factor. this combination increasing runt-related transcription-factor2, osterix, bone alkaline phosphatase, and controls pro-inflammatory cytokines such as interleukin-1beta (IL-1b) and tumor necrosis factor-alpha (TNF-a) so it can enhance bone regeneration and avoid bone destruction. The combination of nHPA-CS, PRF, BFPSC has the potential for alveolar bone regeneration in Aggressive-Periodontitis patients.
