The rise of robotic surgery is transforming medicine. In many ways, urology has taken charge in pioneering a new era of minimally invasive surgery with the emergence of the robot. The unprecedented dissecting precision and the dynamic three-dimensional, high definition view of the surgical field are undoubtedly revolutionizing the field of urology. These unique attributes of robotic surgery confer enormous advantages in dealing with urooncological surgery. The robotic revolution began nearly a decade ago with surgeon-controlled robotic radical prostatectomy and has since expanded to include radical cystectomies and partial nephrectomies. There have been numerous landmark studies published showing that robotic surgery provides comparable oncological and functional outcomes against traditional open or laparoscopic surgery. As a result, it becomes exceedingly imperative that urologists and the oncological community remain up-todate regarding these developments and appreciate the oncological outcomes of surgeon-controlled robotic surgery. This review will assess the impact surgeon-controlled robotic surgery has had in the field of urologic oncology.

N adie cuestiona en 2008 que la vía de elección para la exéresis de una glándula suprarrenal es la laparoscopia 1 . Tampoco se discute el cambio en la cirugía renal que ha supuesto la nefrectomía radical laparoscópica 2 . El acceso laparoscópico ha cobrado por tanto una importancia capital en el manejo de dichas masas, en un intento de disminuir la morbilidad de la lumbotomía.La nefrectomía parcial laparoscópica (NPL) es una técnica en evolución en la mayoría de centros que por un lado compite con la nefrectomía parcial abierta como técnica de elección y por otro lado competirá en un futuro con las técnicas no exeréticas. Estas también se han podido desarrollar de una manera más precisa gracias a la laparoscopia.En este artículo pretendemos, basándonos en una revisión estandarizada de la prolífica literatura a cerca del tratamiento de la MRP, revisar las innovaciones tecnológicas y las mejoras técnicas que la vía de abordaje laparoscópica nos va deparando, no solo como técnica exerética, sino como vehículo de tratamientos mínimamente invasivos, remarcando también los requerimientos de los Servicios de Urología y las mejoras que podrían hacerse en los sistemas sanitarios para que la NPL ganara terreno en el tratamiento de la MRP.La progresiva depuración de la técnica y la mejora del arsenal laparoscópico y de agentes hemostasiantes van a incrementar la indicación de NPL en un futuro. A large number of small renal masses (SRMs) with size less than 4 cm are being identified due to advances in diagnostic imaging. As the natural history of these tumours remains unknown, there is no reliable way to predict their behaviour or future growth. Although, partial nephrectomy is the gold standard for treatment of these tumours, ablative non-surgical therapies such as cryoablation and radiofrequency ablation provide a less invasive option of treatment with comparable oncological outcomes. In this systematic review, the principle, indications, methods of treatment, oncological control, complication and renal function of ablative therapies are critically reviewed. Cryotherapy utilizes the principle of inducing tissue destruction by freezing and thawing using argon and helium gasses, respectively. Radiofrequency ablation (RFA) works on the principle of tissue heating. Ablative treatments are particularly useful in the elderly patients, those with comorbidities or in patients with SRMs in solitary kidneys or renal impairment. Ablative therapies have less procedurerelated complications and have promising medium-term oncological outcome. Longer-term results are accumulating. Cryotherapy may be a better modality for oncological control than RFA. Ablative therapy has emerged as a viable treatment options for SRMs with recurrence free survival rates approaching that of extirpative surgery. However, there is no consensus in the literature on the best selection criteria and this needs further refinement. Prospective long-term data with regards to oncological control is still needed
