Comparison of 1,800 Laparoscopic and Open Partial Nephrectomies for Single Renal Tumors

Abstract: Early experience with laparoscopic partial nephrectomy is promising. Laparoscopic partial nephrectomy offered the advantages of less operative time, decreased operative blood loss and a shorter hospital stay. When applied to patients with a single renal tumor 7 cm or less, laparoscopic partial nephrectomy was associated with additional postoperative morbidity compared to open partial nephrectomy. However, equivalent functional and early oncological outcomes were achieved.

“…With the advent of LPN, Gill and his team analyzed data on 1,800 consecutive open or laparoscopic partial nephrectomies [37]. Gill et al discovered that LPN was highly associated with shorter operative time (P<0.0001), decreased operative blood loss (P<0.0001) and shorter hospital stay (P<0.001) [37].…”

“…Las tasas de complicaciones peroperatorias y de preservación de la función de los riñones afectos (99,6% vs 97,9%) fueron similares entre las dos técnicas. Sin embargo, la NPL se asoció a mayor tiempo de isquemia caliente, mayor número de complicaciones postoperatorias (sobre todo urológicas) y a un mayor número de procedimientos para resolverlas 50 .…”

“…Partial nephrectomy (PN) remains the gold standard for treatment of SRMs with oncological outcome comparable to radical nephrectomy but with better preservation of the renal function [4][5][6]. Laparoscopic partial nephrectomy, however, is technically more demanding with a steep learning curve and has a significant complication rate [7]. Robotic partial nephrectomy has made partial nephrectomy easier [8].…”
