Comparison between the Datex-Ohmeda M-COVX metabolic monitor and the Deltatrac II in mechanically ventilated patients

 RESUMOA monitoração do estado hemodinâmico visa ao equilíbrio entre a oferta e a demanda de oxigênio tecidual. O objetivo deste estudo foi avaliar a utilização da calorimetria indireta (CI) frente à termodiluição (TD) em cães sob diferentes estados hemodinâmicos. Utilizaram-se nove cães pesando 19,6±1,3kg, os quais foram anestesiados com isofluorano a 1,4V% (Basal), submetidos à ventilação mecânica (VM) e estados hipodinâmico (Hipo), com isofluorano a 3,5V%, e hiperdinâmico (Hiper), com dobutamina a 5µg/kg/min. Utilizou-se um cateter de Swan-Ganz para aferição do índice cardíaco (IC)

Relationship between energy expenditure, nutritional status and clinical severity before starting enteral nutrition in critically ill children

The objective of the present study was to investigate the relationship between energy expenditure (EE), biochemical and anthropometric nutritional status and severity scales in critically ill children. We performed a prospective observational study in forty-six critically ill children. The following variables were recorded before starting nutrition: age, sex, diagnosis, weight, height, risk of mortality according to the Paediatric Risk Score of Mortality (PRISM), the Revised Paediatric Index of Mortality (PIM2) and the Paediatric Logistic Organ Dysfunction (PELOD) scales, laboratory parameters (albumin, total proteins, prealbumin, transferrin, retinol-binding protein, cholesterol and TAG, and nitrogen balance) and EE measured by indirect calorimetry. The results showed that there was no relationship between EE and clinical severity evaluated using the PRISM, PIM2 and PELOD scales or with the anthropometric nutritional status or biochemical alterations. Finally, it was concluded that neither nutritional status nor clinical severity is related to EE. Therefore, EE must be measured individually in each critically ill child using indirect calorimetry.

SummaryWe measured oxygen consumption using a new noninvasive modular metabolic monitor, M-COVX TM , in ventilated critically ill patients. Oxygen consumption was measured continuously as part of routine monitoring for up to 24 h following mechanical ventilation in 27 patients admitted to a general intensive care unit. We explored several possible sources of error. Most errors related to inaccurate tidal volume measurement, which resulted in rejection of a median 14% (interquartile range 8-34%) of data. Water accumulation in the pneumotachograph was responsible and occurred more frequently with water bath humidifiers. After manual removal of erroneous data mean oxygen consumption values were virtually identical to calculated values in 24 of 27 patients. We conclude that in most ventilated patients averaging of continuous oxygen consumption data with the M-COVX module results in small errors.
