Comparative Survival Analysis of Porous Tantalum and Porous Titanium Acetabular Components in Total Hip Arthroplasty

Abstract: After adjusting for acetabular defect severity, both PoTa and PoTi acetabular components had excellent survival at mean 44.4-month (range 4.3-91.5 months) follow-up when used in primary and revision THA.

“…Recently, the porous metal has been applied in the design of acetabular cups. 21,22 The porous metal cups always have increased surface roughness than previous cup designs, in order to provide even greater interface friction for further improved initial mechanical stability. 7,33 However, the porous Ti6Al4V fabricated by the conventional technique are oen very brittle and are prone to crack propagation at low stresses, 34 and the pore size, shape, volume fraction, and distribution are difficult to control.…”

“…20 The surface in cementless THA has been subjected to porous surface processing to obtain bone ingrowth, 1 good clinical results and long-term survival rates have been reported for the porous surfaces obtained. 21,22 Many techniques have been investigated to produce porous metallic structure, 23,24 but these conventional techniques result in low porosity and high possibility of pore blockage and have very limited control of the internal pore architecture and external shape of the porous implants. 25 Most current works on porous materials focus on their use as surface coating on the implant to allow bone ingrowth to achieve biologic xation.…”

“…Under the auspices of improved patient care and enhanced outcomes, novel techniques and designs are continuously being introduced in total joint arthroplasty, often with dubious levels of vetting and variable amounts of success. In recent years, minimally invasive surgery 19 , computer assisted surgery 20 , robotic assisted surgery 21 , increased modularity 22 , alternative bearings [23][24][25] , alternative approaches 26 , and alternative fixation 27,28 , have been rapidly adopted into mainstream practice, some with excellent results, and others with unexpected consequences.…”

“…Current research focuses on porous titanium materials characteristics, as well as their comparison with tantalum implants [21,22]. Titanium porous implants have low thermal conductivity, high yield strength, and low weight and have been shown to have high survival [23] and lower cost compared to tantalum implants [14].…”
