The skin microbiome is a complex ecosystem with important implications for cutaneous health and disease. Topical antibiotics and antiseptics are often employed to preserve the balance of this population and inhibit colonization by more pathogenic bacteria. However, despite their widespread use, the impact of these interventions on broader microbial communities remains poorly understood. Here, we report the longitudinal effects of topical antibiotics and antiseptics on skin bacterial communities and their role in Staphylococcus aureus colonization resistance. In response to antibiotics, cutaneous populations exhibited an immediate shift in bacterial residents, an effect that persisted for multiple days posttreatment. By contrast, antiseptics elicited only minor changes to skin bacterial populations, with few changes to the underlying microbiota. While variable in scope, both antibiotics and antiseptics were found to decrease colonization by commensal Staphylococcus spp. by sequencing-and culture-based methods, an effect which was highly dependent on baseline levels of Staphylococcus. Because Staphylococcus residents have been shown to compete with the skin pathogen S. aureus, we also tested whether treatment could influence S. aureus levels at the skin surface. We found that treated mice were more susceptible to exogenous association with S. aureus and that precolonization with the same Staphylococcus residents that were previously disrupted by treatment reduced S. aureus levels by over 100-fold. In all, the results of this study indicate that antimicrobial drugs can alter skin bacterial residents and that these alterations can have critical implications for cutaneous host defense.

Significance and Impact of the Study: The choice of an adequate neutralizing agent is essential for obtaining precise results in the evaluation of disinfectants using the dilution-neutralization method. In this study, we show that the neutralization of triclosan and some of its commercial solutions is not a simple task, requiring complex neutralizers. Moreover, the action of the neutralizer is shown to depend on the micro-organism tested. Specificity in view of the disinfectant and the micro-organism calls for performing the neutralizing assay in each particular case of application of the dilution-neutralization method. We also demonstrate that some of the neutralizers described for triclosan in previous studies are not sufficiently effective. AbstractOne of the most commonly used methods for assessing disinfectants in vitro is the dilution-neutralization test. Although it is known that using an effective neutralizer is essential for obtaining reliable results, in the case of disinfectants like triclosan, whose neutralization is more complicated, an adequate neutralizer is not always used. Here, we compare the efficacy of different neutralizers described previously, and others that have been modified, against several antiseptic formulations of triclosan. The strains of reference used were Pseudomonas aeruginosa, Enterococcus faecium, Staphylococcus aureus and Escherichia coli. The solutions of antiseptic were triclosan 2% aqueous solution, and three alcoholic solutions containing 2, 0Á068 and 0Á5% triclosan, respectively. In the dilution-neutralization test, six neutralizers with different degrees of complexity were assayed. Our results show that the antiseptic solutions studied are effective against the strains of reference, with the exception of triclosan 2% aqueous solution against Ps. aeruginosa. Most neutralizers proved ineffective, depending on the antiseptic solution and the bacterial species, particularly in the case of Staph. aureus. These findings confirm a need to test every neutralizer specifically for each disinfectant, at the maximum concentration used, and for every specific bacterial strain.

RESUMO: O uso de álcool gel em procedimentos de anti-sepsia das mãos representa uma prática freqüente nas instituições de saúde. Assim, estabeleceu-se: determinar in vitro a atividade antibacteriana do álcool gel a 70%, utilizado no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto-USP, frente às bactérias hospitalares e da comunidade, por meio das técnicas de gotejamento e do poço. As bactérias hospitalares foram Staphylococcus aureus, Escherichia coli, Klebsiella spp., Pseudomonas aeruginosa, e da comunidade: S. aureus e Staphylococcus coagulase-negativo. O álcool gel apresentou atividade antibacteriana, pela técnica de gotejamento, frente a todas as bactérias estudadas, porém com a técnica do poço nenhum efeito foi observado. Em conclusão, antes de utilizar alguma substância ou produto contra microrganismos é necessário avaliar previamente a sua eficácia com técnicas ou métodos microbiológicos adequados.
