Objectives To present a comprehensive review of current literature available on Corona virus disease and dentistry, modifications required in dental and laboratory settings; and recommended disinfection protocols in current scenario. Special emphasis has been given to discuss guidelines for handling different prosthodontic procedures and implications of this pandemic on prosthodontic practice, education and research. Materials and methods Relevant literature pertaining to COVID-19 and dentistry was scrutinized on electronic search engines including PubMed, EMBASE and Cochrane. Guidelines given by various organizations, institutions, national and international regulatory bodies and Indian Prosthodontic Society were also consulted to gather information pertaining to the objectives of our review. Results and observations: A total of 160 articles including cross-sectional studies, in vitro study, narrative reviews, letters to the editor and opinions were found to be relevant in accordance with our search strategy. Documented literature revealed that Covid-19 pandemic has culminated in serious clinical, financial and psychological implications in the field of dentistry. Certain steps such as adoption of teledentistry, judicious use of protective equipment, use of rubber dams, pre-procedural rinses have been suggested unanimously. However, there is a dearth of evidence-based recommendations in literature. Data regarding consequences of delaying prosthodontic procedures and patients’ perspectives is also sparse. Conclusion The COVID-19 necessitates the need to adopt a balanced approach while treating patients and safeguarding the dental professionals at the same time. Risk-benefit ratio has to be assessed along with stringent following of guidelines and disinfection protocols to combat this unprecedented situation.

Hospital-acquired infections (HAIs) are one of the most tangled difficulties in advanced clinical practices. These infections lead to financial implications and have a significant impact on morbidity and mortality. It is very difficult to eradicate the HAIs however both disinfection and sterilization account for the best measure to control HAIs. Of all the disinfectants in use, one of the chemical disinfectants which seem to be user-friendly, non-corrosive, and used extensively are Quaternary ammonium compounds (QACs) based disinfectants. To improve the efficacy of these disinfectants and tackle the challenge of antimicrobial resistance (concerning QACs), from time to time newer QACs disinfectants were introduced which are termed as first, second, third, fourth, and fifth-generation QACs disinfectants. Manufacturers of these newer generations QACs disinfectants claim these compounds as high-level, broad-spectrum disinfectants while leading healthcare agencies like the Centre for Disease Control & Prevention (CDC) mention QACs as mild disinfectants and not sporicidal. Sadly, the antimicrobial efficacy of QACs has been largely assessed using old methods like phenol coefficient methods & suspension methods, and not using an internationally standardized method. These loopholes raise a lot of queries about the true efficacies of the QACs and thus, increase the chances of the development of resistant HAIs. Therefore, there is an urgent need for better and standardized methods to study the efficacy of different generation QACs. The present review discusses the status of currently available methods and gaps in the literature that would be useful to highlight the potential use of QACs for infection control and prevention in better ways.

BACKGROUNDIntensive care units (ICUS), High dependency units (HDUS) and Isolation units or wards carry high risk of nosocomial infection, morbidity, mortality and health care cost. To limit the nosocomial infection, health care providers should adopt aggressive and effective infection control measures. The importance of disinfection is frequently acknowledged but previous data indicated little distinction between manual and automated area decontamination. WHO has updated guidelines on infection control from time to time. MATERIALS AND METHODSVarious studies recommended automated fogging system for environmental decontamination and with this perspective we conducted our study using fogger with three different disinfectants namely 1) H2O2 silver solution; 2) Octyldecyldimethylammonium chloride, Dioctyldimethylammonium chloride, Didecyldimethylammonium chloride, Alkyldimethylbenzylammonium chloride; 3) N-Alkyldimethylbenzylammonium chloride, Didecyldimethylammonium chloride, Polymeric biguanide hydrochloride. During the study, rooms of isolation wards, high definition units and intensive care units were fogged separately with all the three disinfectants along with high end cleaning of all the surfaces. The swab samples were taken from various sites like wall, bed, floor and air. RESULTSThe results of growth then obtained were compared and analysed. The causes of bacterial and fungal growth even after fogging were analysed and enumerated. The control measures were adopted to reduce bacterial and fungal growth to nil. CONCLUSIONInfection control policy and guidelines were prepared and distributed to concerned departments for implementation and to check cross infection at various vulnerable points.
