Amaç Bu çalışmada, S.aureus izolatlarının metisilin, yüksek düzey mupirosin ve fusidik asite karşı antibiyotik direnç özelliklerinin fenotipik ve genotipik olarak açığa çıkarılması amaçlanmıştır. Gereç ve Yöntemler: Antibiyotik direnç özelliği bilinmeyen 100 farklı Staphylococcus.aureus (S.aureus) izolatı klinik örneklerden saflaştırılmış ve bu izolatların metisilin, yüksek düzey mupirosin ve fusidik asit antbiyotik direnci ve plazmit içerikleri belirlenmiştir. Metisilin, yüksek düzey mupirosin ve fusidik asit duyarlılıkları phoenixsistemi ile belirlenmiştir. Ayrıca Metisilin direnç özelliği 30 µg sefoksitin diski kullanılarak disk difüzyon yöntemi ile de araştırılmıştır. İzolatlardaki antibiyotik direnç özelliği ile ilgili mecA, ileS-2, fusA, fusB ve fusC genlerinin varlığı ise PCR amplifikasyonu ile araştırılmıştır. Bulgular: Disk difüzyon ve phoenix analiz sonuçlarına göre tanımlanan S. aureus izolatlarınının sefoksitin direnci %19 olarak tespit edilmiştir. Phoenix otomotize sistem ile yüksek düzey mupirosin ve fusidik asit direnci tespit edilmemiştir. Moleküler analiz sonuçlarına göre ise mecA, fusA ve fusC genlerinin izolatlardaki frekansı sırası ile %19, %98 ve %1 olarak belirlenmiş fakat ileS-2 ve fusB genleri amplifiye edilmemiştir. Antibiyotik direnç ve plazmit varlığı arasındaki ilişkinin belirlenebilmesi için izolatların plazmit varlığı belirlenmiş ve izolatların %79'unda farklı sayılarda plazmit içerdiği tespit edilmiştir. Sonuç: Fenotipik ve genotipik yöntemle elde edilenlere sonuçlar benzerlik göstermesine karşın benzer olmayan bazı sonuçları da içermektedir. Stafilokok enfeksiyonlarının tedavisinde, antibiyotik direnç genlerinin doğru ve hızlı bir şekilde teşhisi, enfeksiyonların yayılmasının önlenmesinde çok önemlidir. Antibiyotik dirençinin doğru saptanması için fenotipik yöntemler ile birlikte PCR bazlı moleküler yöntemler de tercih edilmelidir.

Background: Diabetic foot infection (DFI) is a common and costly complication of diabetes that may be caused by various bacteria with multi-resistant genes. The aim of this study is to evaluate the efficacy of phenotypic methods for identification of methicillinresistant Staphylococcus aureus (MRSA) with genotypic detection of MRSA-related genes. Methods: In this cross-sectional study, swab samples were collected from patients with DFI from hospitals in Sulaimani/Iraq in April-July 2019. All the samples were processed for microbiological assessment and further MRSA phenotypic and genotypic testing. Results: A total of 46 swab samples were collected from diabetic foot ulcers of 29 males and 17 females. Most samples (93.5%) showed positive growth, with higher proportions of monomicrobial (23; 53.5%) than mixed-bacterial infections (20; 46.5%) and S. aureus as the predominant pathogen. Conventional methods of MRSA detection, such as cefoxitin disc diffusion, can predict methicillin resistance in 45.8% of the cases. Real-time/conventional PCR showed that 41.6% of Staphylococcus aureus were positive for the mecA gene, while none of the isolates was positive for PVL. Conclusion: Staphylococcus aureus was the predominant pathogen in DFI. Although cefoxitin and oxacillin disc diffusion methods can help in the prediction of MRSA, realtime PCR is a reliable method for MRSA detection and confirmation.

Nasal carriage of Staphylococcus aureus (S. aureus) probably causes the transmission of infection between individuals in hospital and community. This study aimed to evaluate the molecular epidemiology and antibiotic resistance pattern of nasal carriage S. aureus in pediatric ward patients and personnel. A total of 122 Nasal samples were taken from 28 personnel and 94 hospitalized patients in the pediatric ward. Minimum Inhibitory Concentration (MIC) to vancomycin and cefoxitin was determined by Agar dilution method strips. All S. aureus isolates were analyzed by pulsed-field gel electrophoresis (PFGE). A total of 41 S. aureus were isolated from the patients. 16 isolates (39.09%) were hospital-associated S. aureus (HA-SA) and 25 (60.97%) were community-associated S. aureus (CA-SA); also, 13 S. aureus isolates were obtained from the personnel. Based on MIC results, all of S. aureus isolates were susceptible to vancomycin, and in 41 patient isolates, 13 isolates (31.7%) were resistant to cefoxitin (MRSA). Of 13 S. aureus isolates of the personnel, 3 (23%) isolates were MRSA. Totally 11 common clones and 13 single clones were obtained. In conclusion the prevalence of CA-SA in the ward was higher than that of HA-SA. In the strains obtained from a hospital ward, there was a high epidemiology, genotypic diversity in the studied ward. However, horizontal transfer of S. aureus was observed between patients and between personnel and patients, which indicated the risk of transmission of resistant strains in the hospital wards.

Abstract: Results of oxacillin screen agar and cefoxitin DD test were in concordance with mecA gene PCR. Thus, it is determined that especially cefoxitin test can be an alternative to PCR in routine.
