Combined Immunohistochemistry for the “Three 7” Markers (CK7, CD117, and Claudin-7) Is Useful in the Diagnosis of Chromophobe Renal Cell Carcinoma and for the Exclusion of Mimics: Diagnostic Experience from a Single Institution

Abstract: Background There is a morphological overlap among renal epithelial tumors, particularly chromophobe renal cell carcinoma (CHRCC), clear cell renal cell carcinoma (CCRCC), renal oncocytoma (RO), and papillary renal cell carcinoma (PRCC). Discriminating between these tumors is important but sometimes challenging. This study is aimed at evaluating the clinical usefulness of the combined immunochemistry for the “three 7” markers (CK7, CD117, and Claudin-7) to distinguish chromophobe renal cell carcinoma from these… Show more

“…The feature of upgraded nuclei is consistent with the increased eosinophilic cytoplasm in CCRCC and PRCC, but this feature was not appreciated in the JCTs 24 . The expressions of epithelial‐ and RCC‐related markers can easily differentiate these conditions 6,14 . Applications of FISH may be helpful in the differential diagnoses in the exceptionally difficult cases.…”

“…The probes included CSP3+GSP 3p (LBP, Guangzhou, Guangdong, China), CEP7, CEP17, SEY (Vysis, Downers Grove, IL, USA), dual‐color break‐apart TFE3 (LBP, Guangzhou) and TEB probes (LBP, Guangzhou). The procedures and the cut‐off for the positive signals have been previously described in other studies 3‐8 . The fresh samples from five cases (case 7, 8 and 16–18) were available for ultrastructural analysis by using a transmission electron microscope (JEOL‐1011; JEOL, Ltd, Tokyo, Japan).…”

“…IHC and special stain markers for distinguishing chRCC and oncocytoma include CK7, S100A1, CD117 (CKIT), kidney specific cadherins, KAI, cyclin D1, and Hale's colloidal iron are currently used to differentiate the tumors in discrepant cases. CK7 has a wide range of sensitivity and specificity, ranging from 73-100% and 71-100%, respectively [41][42][43][44][45] in different studies. Similarly, S100A1 has sensitivities and specificities at 80-100% and 70-92%, respectively [11,46].…”

“…The second most common immunohistochemical marker for the differential diagnosis of oncocytic renal cell neoplasms is CD117. However, CD117 does not discriminate between cRCC and rO as about 85% are positive [40], but this marker is useful to discriminate cRCC and rO from oncocytic papillary renal cell carcinoma, oncocytic angiomyolipoma and others [1].…”
