Combined fractures of the humeral head and the glenoid

Abstract: Background. Glenohumeral combination fractures are severe shoulder injuries for which, however, few data are available in the literature. The aim of the study was to analyze the incidence and numeric distribution of glenohumeral combination fractures using a classification system. Furthermore, treatment methods and complications are discussed. Methods. This retrospective study is based on the data evaluation of a level I trauma center between 1998 and 2016. Glenohumeral combination fractures are classified int… Show more

“…Rarely, proximal humerus fractures are associated with glenoid fractures. This combination may be indicative of an instability event at the time of injury and is often treated surgically [ 7 ], especially when the glenoid fragment is displaced. Although there is no universal consensus for the surgical indications to stabilise these fractures, Königshausen et al have proposed that surgery is indicated when the glenoid fragment presents with a medial displacement of more than 3 mm and/or a fracture gap of more than 5 mm [ 7 ].…”

“…Glenohumeral combination fractures were described as an entity and classified by Könignhausen et al [ 7 ], who described four possible types. Type 1b describes a large glenoid rim fracture, a greater tuberosity fracture, and an additional coracoid process fracture.…”

“…fractures into groups according to the severity of each fracture, and suggested a treatment algorithm for these injuries, predominantly surgical treatment 18 . Our cohort of patients would correspond to their classification of Type 2 glenohumeral combination fractures, describing them as large anterior glenoid rim fracture and proximal humeral fracture.…”

“…Anterior shoulder instability is frequently associated with glenoid bone loss. 9,14,20,25 If the glenoid bone loss exceeds a certain amount, soft tissue stabilization surgery often does not provide sufficient stability. 9 The cutoff value for critical glenoid bone loss that necessitates surgical treatment remains a subject of debate.…”
