Combined Esophageal Multichannel Intraluminal Impedance and pH Monitoring (MII -pH) in the Diagnostics and Treatment of Gastroesophageal Reflux Disease and its Complications

Abstract: The technique of 24-hour esophageal multichannel intraluminal impedance monitoring combined with pH-metry (MII-pH) is currently considered to be the golden standard in the diagnostics of gastroesophageal reflux disease (GERD). The technique allows for differentiation of gas and liquid reflux as well as detection of non-acid reflux, which cannot be detected with other techniques that are based only on measuring the pH of gastric contents.THE AIM OF THE STUDY was to assess the usefulness of MII-pH in the diagnos… Show more

“…The diagnostic power of combined MII and 24-hour pH monitoring in detecting all refluxes and acid refluxes in infants were statistically higher than pH monitoring alone in children older than one year. Adding the MII to 24-hour pH monitoring method increased the accuracy of the detection of reflux events (4,8) . There was a relationship between reflux and atypical symptoms, particularly respiratory ones and also regardless of age and the relationship between reflux and atypical symptoms in infants less than one year of age (5) .…”

“…Although the pH-24h may be improved with impedance reflux monitoring by increasing the understanding of the pathophysiology and etiology of GERD to establish differential diagnoses 12 , the use of both tests may not be feasible considering the costs and accessibility of the examinations in most Brazilian territory. The use of pH-24h alone should be considered a mandatory examination for the precise diagnosis of GERD and in the preparation of patients for surgical treatment, for reasons described as follows: The use of pH-24h may avoid the issues concerning the use of PPIs in both diagnostics and treatment.…”

“…The best way of reflux testing is combined intraluminal pH-impedance. It can be performed also in patients undergoing therapy [13,14]. Analysis of pH monitoring (MII-pH) can lead to 4 different phenotypes in patients.…”
