Combined cataract and glaucoma surgery: trabeculectomy versus endoscopic laser cycloablation

Abstract: Endoscopic laser cycloablation performed through a cataract incision was a reasonably safe and effective alternative to combined cataract and trabeculectomy surgery, providing an option for cataract patients who have glaucoma requiring surgical intervention.

“…In our study, the overall success rate was 92.57% and 78.84% at 1 and 5 years of follow-up, respectively. Gayton et al (13) compared cataract surgery combined with trabeculectomy or ECP. The ECP cases did not achieve an immediate reduction in IOP, as the trabeculectomy cases did.…”

“…Gayton JL et al 73 conducted a randomized prospective study on 58 eyes of 58 patients comparing endoscopic laser cycloablation performed through a cataract incision at the time of cataract surgery with combined trabeculectomy and cataract surgery. At the final available visit of 2 years follow-up, 30% of endoscopic laser patients achieved intraocular pressure control (below 19 mm Hg) without medication and 65% with medication.…”

“…Researchers have proved Combined surgery is better than trabeculectomy alone because it manages both diseases at the same time and gives desirable results in terms of IOP control and visual outcome. 6 Of the various glaucoma surgeries trabeculectomy is the best technique to be used in association with cataract extraction. 7 The protective scleral flap over a limbal fistula reduces the chances of an early postoperative flat anterior chamber and also makes the guarded filtering operation particularly desirable for combined procedures.…”
