Detection of Cryptosporidium spp. in Diarrheic Immunocompetent Patients in Beni-Suef, Egypt: Insight into Epidemiology and Diagnosis

Cryptosporidium species is an important cause of gastrointestinal infections globally. This study aimed to shed light on its role in diarrheic immunocompetent patients in Beni-Suef, Egypt and to compare three diagnostic methods. Two hundred diarrheic patients, 37±16.8 year old, were enrolled. Stool samples were examined by light microscopy, using modified Ziehl-Neelsen stain (MZN) for Cryptosporidium spp. oocysts. Coproantigens were detected by sandwich ELISA. DNA molecular diagnosis was done by nested PCR. PCR yielded the highest detection rates (21.0%), compared to ELISA (12.5%) and MZN staining method (9.5%). The higher infection rates were in 20–40 year-old group, followed by 40–60 year-old. Association between epidemiologic factors was statistically not significant; positivity and gender, clinical manifestations, residence, source or water, or contact with animals. Cryptosporidiosis is an important enteric parasitic infection in Beni-Suef and PCR remains the gold standard for diagnosis.

A 106 different specimen were collected to isolate and diagnose some gram negative and gram-positive bacteria, as it was performed for the period from 15 August to 30 November 2019 from AL-Salam and Al-Khansaa educational hospitals in Mosul. Lipopolysaccharide (LPS) was extracted from Enterobacter cloacae by using the Methanol-Chloroform method. extract was diagnosed, using some biochemical tests such as the Molisch test to detect carbohydrate content and Folin reagent in order to determine proteins concentration, lipid solubility in organic solvent and Bio drop technique as well as to detect nucleic acid concentration. The extraction methods showed its efficiency with low protein and nucleic acids concentration in the extract. Lipopolysaccharide demonstrated its efficiency inhibiting microbial growth on all test isolates at concentrations 1100, 1000 µg\ ml while its lipopolysaccharide showed its inhibition of Proteus mirabilis at all concentrations used. LPS showed possibility used as inhibitory, antimicrobial growth.

The Washington State Department of Health Public Health Laboratories (WAPHL) has tested 11,501 samples between 2007 and 2017 for a foodborne disease using a combination of identification, serotyping, and subtyping tools. During this period there were 8037 total clinical and environmental samples tested by pulsed-field gel electrophoresis (PFGE), including 512 foodborne disease clusters and 2176 PFGE patterns of Salmonella enterica subsp. enterica . There were 2446 Shiga toxin–producing Escherichia coli samples tested by PFGE, which included 158 foodborne disease clusters and 1174 PFGE patterns. There were 332 samples of Listeria monocytogenes tested by PFGE, including 35 foodborne disease clusters and 104 PFGE patterns. Sources linked to outbreaks included raw chicken, unpasteurized dairy products, various produce types, and undercooked beef among others. As next-generation sequencing (NGS) replaces PFGE, the impact of this transition is expected to be significant given the enhanced cluster detection power NGS brings. The measures presented here will be a reference baseline in future years.
