Colonic duplication in an adult mimicking a tumor of pancreas

Abstract: Duplications of the alimentary tract are uncommon congenital malformations that can present diagnostic difficulties. We report a rare case of a cystic colonic duplication in a female adult. Preoperative investigations were suggestive of pancreatic tumor. The diagnosis was established based on the histopathological examination of the resected specimen. We concluded that, though uncommon, intestinal duplication should be considered in differential diagnosis of abdominal mass.

“…Histological analysis typically reveals at least one outer muscular layer with an inner gastrointestinal mucosal lining. Gastric mucosa may be found in over 50% of duplications including the colon and rectum 1 3 6. Ectopic gastric mucosa was found in only one case in our series.…”

“…CT is the most suitable diagnostic method for detecting digestive duplication, 15 because it is easy to perform, especially in pediatric and critically ill patients. Cystic duplication can be mistaken for a cystic tumor of adjacent organs, 4 , 14 especially in patients in whom luminal communication does not exist. Sometimes the correct diagnosis can only be made during exploratory laparotomy.…”

“…If colonoscopy is performed in this case, the presence of huge impacted fecaloma might obscure visualization of the orifice. CT scan is helpful in the diagnosis of colonic duplication but some literature have reported that duplication cyst in adults mimics other conditions such as pancreatic tumor [ 9 ], mesenteric inflammatory mass, or Meckel's diverticulum [ 10 ]. Various authors have agreed that the diagnosis of colonic duplication is difficult and is not usually made preoperatively [ 9 ].…”

In the present case, CT scan was suggestive of small bowel duplication and the correct final diagnosis was only made intraoperatively.…”
