Colitis Cystica Profunda Simulating Rectal Carcinoma

Abstract: Localized colitis cystica profunda is a rare, benign disease of the lower gastro-intestinal tract, usually presenting as a rectal mass and characterized microscopically by the presence of mucus-filled cysts in the submucosa. Knowledge of this particular pathological entity is important as it can mimic a well differentiated adenocarcinoma of the rectum and therefore could lead to unnecessary surgical resection. We present a case of colitis cystica profunda misdiagnosed as adenocarcinoma based on their similar c… Show more

“…For example, colitis cystica profunda is characterized by misplacement of mucin-filled crypts through the muscularis mucosae into the submucosa and muscle layer of the colon. 12 Extracellular mucin occurs in some cases mimicking mucinous adenocarcinoma of the colon. A similar lesion has been described in the solitary rectal ulcer syndrome and has been misinterpreted as adenocarcinoma of the rectum.…”

“…La ecoendoscopia nos puede ayudar a diferenciar la enfermedad del cáncer, visualizando quistes hipoecoicos en la submucosa, sin infiltración de las capas más profundas, ni de los ganglios locales, descartando así el cáncer. La TAC y la resonancia magnética también pueden apoyar el diagnóstico (2,3,8,9).…”

“…CCP is a rare benign disease characterized by mucin-filled cysts beneath the muscularis mucosa [1]. Knowledge of this pathological entity is important as it can mimic a well-differentiated adenocarcinoma of the rectum and therefore could lead to unnecessary surgical resection [2]. EUS, computed tomography, and MRI are useful for diagnosis of CCP [3,4]; however, the definitive diagnosis is based on endoscopic biopsies, which should be large and deep enough to include the submucosal layer, or resected surgical E-Videos ▶ Fig.…”
