Coat’s-like exudation in rhodopsin retinitis pigmentosa: successful treatment with an intravitreal dexamethasone implant

Abstract: Purpose Role of intravitreal injection of dexamethasone implant (Ozurdex) for refractory macular thickening. Methods A case report of a 13-year-old boy with Rhodopsin-positive, CRB1-negative retinitis pigmentosa presenting with Coat's-like exudative vitreoretinopathy and treatment-resistant cystoid macular oedema. Results A reduction in the macular thickening following a single injection of Ozurdex. Conclusion We present our experience in successful treatment of refractory macular oedema with intravitreal inje… Show more

“…Searching the literature for suggested management of patients having coat’s like disease and RP revealed multiple management plans including: vitrectomy and endolaser [ 9 ], cryotherapy, scleral buckling with subretinal fluid drainage and laser photocoagulation of the telangiectatic and neovascular lesions [ 10 ] and intravitreal dexamethasone implant [ 11 ].…”

“…The studies reporting on the use of DEX in macular edema related to retinitis pigmentosa consist of case report studies, which include only a few eyes ( Table 7 ). 77 80 Nevertheless, an anatomical and functional improvement has been shown, but a relapse of macular edema occurred before 6 months from the implant 77 and an additional DEX was required in some cases. 77 , 78 DEX implant proved to be safe with an IOP rise >21 mmHg recorded in only one eye.…”

“… 33 Despite increasing availability of intravitreal agents, management continues to be an ongoing challenge with heterogeneous visual and anatomical outcomes. 34 In our report, disease control was achieved with laser photocoagulation in combination with intravitreal therapy. Interestingly, macular edema in our patient seemed to respond better when treated with triamcinolone than when bevacizumab was used.…”
