CO2Laser Treatment of Epidermal Nevi: Long-Term Success

Abstract: Carbon dioxide laser vaporization of epidermal nevi provides good clinical effect and offers unique advantages for the treatment of these lesions, including effective intraoperative hemostasis with excellent lesional visualization. It is also possible to treat widespread areas in one laser treatment session. While the results of this series clearly show the benefit of CO2 laser treatment, epidermal nevi may not always respond so favorably, due in part to the variability in their depths of involvement.

“…There are few reports on laser treatment of EN. Various studies report good clinical effect when using CO 2 laser for the treatment of epidermal nevi [6,7,8,9]. Low recurrence rates have been reported, perhaps due to the creation of a limited and protective amount of dermal fibrosis that occurs as a result of laser skin resurfacing [7].…”

“…Topical modalities, including steroids (topical or intralesional), retinoids, calcipotriol, toxins (e.g. podophyllin), antiproliferatives (5-fluorouracil cream), keratolytics, and chemical peels, have shown inconsistent efficacy and a high rate of recurrence, and destructive methods, including laser, electrocoagulation, and dermabrasion, have not uniformly provided acceptable efficacy and side effects and a high rate of recurrence [3,4,5,6,7,8,9,10,11]. Surgery is an option for small lesions, but in large lesions the scar is not necessarily a worthwhile trade-off.…”

“…The zonal creation of fibrosis in the area treated with ablation may minimize the risk of recurrence. 18 The primary thickness of the nevi is the most determining factor for cosmetic results. 19 We report a patient with profound expression of the neurological and oropharyngeal complications of linear epidermal nevus, an entity within the spectrum of epidermal nevus syndrome.…”
