Co‐Occurrence and Characteristics of Patients With Axial Spondyloarthritis Who Meet Criteria for Fibromyalgia

Abstract: Objective To estimate the proportion of patients with axial spondyloarthritis (SpA) in a UK national biologics registry who met criteria for fibromyalgia (FM), and to delineate the characteristics of these patients. Methods Two cohorts of patients are prospectively recruited from across 83 centers in the UK for the British Society for Rheumatology Biologics Register in Ankylosing Spondylitis (BSRBR‐AS). All patients are required to meet Assessment of SpondyloArthritis international Society (ASAS) criteria for … Show more

“…The prevalence of depression reported in our cohort was very similar to results from a prior meta-analysis in axSpA (16% vs 15%) [25]. Fibromyalgia (5%) was likely under-diagnosed, compared to prevalence reported using meta-analysis (13%) and found by screening questionnaire in the British Society for Rheumatology Biologics Register for AS (21%) [26,27]. Associations between clusters dominated by these two conditions and patient-reported outcome measures were similar in magnitude to those reported in other studies [25][26][27]7].…”

Associations between clusters dominated by these two conditions and patient-reported outcome measures were similar in magnitude to those reported in other studies [25][26][27]7]. In previous meta-analysis, depression was additionally associated with objective measures such as ESR and the Bath AS metrology index [25].…”

“…Future refinements and abbreviations of this tool will hasten translation. Moreover, since coexisting FM is a common issue among many diseases (musculoskeletal and beyond) , such a tool may be generically applicable, and therefore testing across the spectrum of chronic pain disorders may finally move the pain field into the era of personalized medicine.…”

“…In addition, some medications may contribute to pain, such as statins, aromatase inhibitors, bisphosphonates, and opioids (ie, opioid-induced hyperalgesia). However, these conditions and many others (eg, rheumatoid arthritis, osteoarthritis, systemic lupus erythematosus [SLE], spinal stenosis, neuropathies, Ehlers Danlos syndrome, 51 sleep disorders such as sleep apnea, and mood and anxiety disorders 121 ) also co-occur in patients with FM. The clinician must determine the possible contribution of various disorders to the patient's presentation.…”
