AAPT Diagnostic Criteria for Fibromyalgia

Abstract: Fibromyalgia (FM) is a common chronic pain disorder that presents diagnostic challenges for clinicians. Several classification, diagnostic and screening criteria have been developed over the years, but there continues to be a need to develop criteria that reflect the current understanding of FM and are practical for use by clinicians and researchers. The Analgesic, Anesthetic, and Addiction Clinical Trial Translations Innovations Opportunities and Networks (ACTTION) public-private partnership with the U.S. Foo… Show more

“…FM is characterized by a constellation of symptoms that include chronic pain, fatigue, distressed mood, and sleep dysfunction . Although there are ongoing efforts to accurately diagnose persons with FM, at present, the diagnosis of FM is assessed using the 2016 revised FM criteria (American College of Rheumatology‐2016), which is based on the Fibromyalgia Research Criteria . Given the level of subjectivity in the diagnostic rubric for FM, the recognition of objective key causal factors/mechanisms leading to the development or confirmed diagnosis of FM and FM‐related symptoms, particularly pain, and objective measures to confirm FM diagnosis have not been identified .…”

“…Based on previous reports emphasizing the frequent coexistence of either fibromyalgia (FM) and chronic fatigue syndrome (CFS) with migraine and the possibility of common pathophysiological mechanisms among these disorders, the presence of FM and CFS was also noted. The diagnosis of FM was made according to ACTTION-APS Pain Taxonomy diagnostic criteria [15], and the diagnosis of CFS was made based on the National Institute for Health and Care Excellence guidelines [16]. The presence of symptoms of cognitive disturbances was also noted.…”

“…In 2016, Wolfe et al published a revision of the 2010/2011 criteria, which was supposed to diminish the misclassification of regional pain disorders (Wolfe et al, 2016). In 2018, Arnold et published new criteria (Arnold et al, 2018), which were subsequently criticized by Wolfe, (2019).…”
