Health is gold, and good health is a matter of survival for humanity. The development of the healthcare industry aligns with the development of humans throughout history. Nowadays, along with the strong growth of science and technology, the medical domain in general and the healthcare industry have achieved many breakthroughs, such as remote medical examination and treatment applications, pandemic prediction, and remote patient health monitoring. The advent of 5th generation communication networks in the early 2020s led to the Internet of Things concept. Moreover, the 6th generation communication networks (so-called 6G) expected to launch in 2030 will be the next revolution of the IoT era, and will include autonomous IoT systems and form a series of endogenous intelligent applications that serve humanity. One of the domains that receives the most attention is smart healthcare. In this study, we conduct a comprehensive survey of IoT-based technologies and solutions in the medical field. Then, we propose an all-in-one computing architecture for real-time IoHT applications and present possible solutions to achieving the proposed architecture. Finally, we discuss challenges, open issues, and future research directions. We hope that the results of this study will serve as essential guidelines for further research in the human healthcare domain.

There is a massive transformation in the traditional healthcare system from the specialist-centric approach to the patient-centric approach by adopting modern and intelligent healthcare solutions to build a smart healthcare system. It permits patients to directly share their medical data with the specialist for remote diagnosis without any human intervention. Furthermore, the remote monitoring of patients utilizing wearable sensors, Internet of Things (IoT) technologies, and artificial intelligence (AI) has made the treatment readily accessible and affordable. However, the advancement also brings several security and privacy concerns that poorly maneuvered the effective performance of the smart healthcare system. An attacker can exploit the IoT infrastructure, perform an adversarial attack on AI models, and proliferate resource starvation attacks in smart healthcare system. To overcome the aforementioned issues, in this survey, we extensively reviewed and created a comprehensive taxonomy of various smart healthcare technologies such as wearable devices, digital healthcare, and body area networks (BANs), along with their security aspects and solutions for the smart healthcare system. Moreover, we propose an AI-based architecture with the 6G network interface to secure the data exchange between patients and medical practitioners. We have examined our proposed architecture with the case study based on the COVID-19 pandemic by adopting unmanned aerial vehicles (UAVs) for data exchange. The performance of the proposed architecture is evaluated using various machine learning (ML) classification algorithms such as random forest (RF), naive Bayes (NB), logistic regression (LR), linear discriminant analysis (LDA), and perceptron. The RF classification algorithm outperforms the conventional algorithms in terms of accuracy, i.e., 98%. Finally, we present open issues and research challenges associated with smart healthcare technologies.

Internet of Things (IoT) is finding application in many areas, particularly in health care where an IoT can be effectively used in the form of an Internet of Medical Things (IoMT) to monitor the patients remotely. The quality of life of the patients and health care outcomes can be improved with the deployment of an IoMT because health care professionals can monitor conditions; access the electronic medical records and communicates with each other. This remote monitoring and consultations might reduce the traditional stressful and costly exercise of frequent hospitalization. Also, the rising costs of health care in many developed countries have influenced the introduction of the Healthcare Monitoring Application (HMA) to their existing health care practices. To materialize the HMA concepts for successful deployment for civilian and commercial use with ease, application developers can benefit from a generic, scalable framework that provides significant components for building an HMA. In this chapter, a generic maintainable HMA is advanced by amalgamating the advantages of event-driven and the layered architecture. The proposed framework is used to establish an HMA with an end-to-end Assistive Care Loop Framework (ACLF) to provide a real-time alarm and assistance to monitor pregnant women.
