SummaryObjective There is limited information about the prevalence of vitamin D deficiency and the effects of treatment on immigrants. The effects of oral vitamin D intake and UVB treatment on vitamin D status in healthy Somali women living in Sweden were analysed. Design Two studies were carried out; a randomized, doubleblind, placebo-controlled study, with oral drops of 800 IU and 1600 IU cholecalciferol and similar amounts of placebo given daily during 12 weeks and a single-blind, placebo-controlled study, using UVB (4Á3-8Á7 J/cm 2 ) or Woods lamp (placebo) on the upper body, or the face and hands.Patients One-hundred fourteen Somali women, mean age 34 years, latitude 0-10°N, living in Sweden >2 years, latitude 57°N, participated. Measurements Serum 25-hydroxyvitamin D (S-25(OH)D) was monitored before, every 6 weeks and at 3 months after treatment.Results The majority of the women (n = 83, 73%) were vitamin D-deficient, S-25(OH)D < 25 nmol/l at start. There was a dose-dependent increase in S-25(OH)D levels (P = 0Á001, stratified Jonckheere-Terpstra test) with a mean increase after twelve weeks in women treated with 800 IU/day and women treated with 1600 IU/day of 18 nmol/l (95% CI: 6-29, median = 17) and 29 nmol/l (95% CI: 17-42, median = 34), respectively. S-25 (OH)D decreased during follow-up but remained above baseline levels. The placebo group remained unchanged throughout the study. UVB treatment increased S-25(OH)D dose-dependently after 6 weeks (P = 0Á03, Jonckheere-Terpstra test).Conclusions Vitamin D deficiency was common in immigrants living at higher latitudes. Vitamin D treatment increased S-25 (OH)D levels dose-dependently during 3 months. The effect was maintained for another 3 months. At least 1600 IU/day is recommended. The dropout rate was high.

Introduction: Vitamin D is a hormone responsible in the regulation of immune response. This study was designed to assess the relationship of vitamin D deficiency on the development of infections in patients with primary immunodeficiency (PID). Materials and Methods:A total of 39 patients (M/F: 23/16) and 39 age-and sex-matched healthy controls (M/F: 23/16) were enrolled in the study. We measured the frequency of infections and serum vitamin D levels in winter and summer seasons. Results:The median age of the patient and control groups were 15.0 (Interquartile range: 7.5-26.0) years and 14.0 (9.0-25.0) years, respectively (p=0.810). Winter and summer vitamin D levels were similar both in patient and control groups (p=0.492 for winter, p=0.503 for summer). Number of the patients with low serum vitamin D levels and with infections were higher in winter than in summer. Autoimmune and inflammatory diseases (AID) were predominantly observed in patients with low winter vitamin D levels and in common variable immunodeficiency (CVID) group. Patients with CVID had lower serum vitamin D levels both in summer (p=0.048) and in winter (p=0.008). Conclusions:In this study, we showed the seasonal variation of serum vitamin D in patients with PID. There was also increased frequency of CVID and AID in those patients with low vitamin D levels. In addition, vitamin D might be given in winter to the patients with PID to take infections and autoimmune disorders under control. ÖzGiriş: D vitamini, bağışıklık cevaplarının düzenlenmesinden sorumlu bir hormondur. Bu çalışma, D vitamini eksikliğinin primer immün yetersizliği olan hastalarda (PİY) enfeksiyon gelişimi ile ilişkisini değerlendirmek için tasarlanmıştır.Gereç ve Yöntemler: Çalışmaya toplam 39 hasta (E / K: 23/16) ve 39 yaş ve cinsiyet uyumlu sağlıklı kontrol grubu (E / K: 23/16) alındı. Kış ve yaz aylarında enfeksiyon sıklığı ve serum D vitamini düzeyleri saptandı.Bulgular: Hasta ve kontrol grubunun ortanca yaşı 15.0 (çeyrekler arası aralık: 7.5-26.0) ve 14.0 (9.0-25.0) idi (p = 0.810). Kış ve yaz D vitaminleri hem hasta hem de kontrol grubunda benzerdi (p=0.492 kış için, p0.503 yaz için). Serum D vitamini düzeyi düşük ve enfeksiyonu olan hastaların sayısı kışın yaz aylarında olduğundan daha fazla idi. Otoimmün ve enflamatuvar hastalıklar (OIH) ağırlıklı olarak, düşük kış D vitamini düzeyi olan hastalarda ve yaygın değişken immün yetersizlikli hastalık (YDİY) grubunda gözlemlendi. YDİY tanılı hastaların hem yaz (p=0.048) hem de kış (p=0.008) D vitamini düzeyleri düşüktü.Sonuç: Bu çalışmada PİY tanılı hastalarda serum D vitamini düzeylerinin mevsimsel değişimi gösterilmiştir. Düşük D vitamini düzeyleri olan hastalarda ayrıca YDİY ve OİH sıklığı artmıştır. Objective:The extent of severity and complexity of coronary artery disease (CAD) in patients presenting with ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) and possible correlations between serum 25-hydroxyvitamin D (25(OH)D) have not yet been adequately studied. We evaluated the relationship between 25(OH)D levels and the burden of CAD as assessed by the SYNTAX score (SXscore) in patients with acute coronary syndrome (ACS) including STEMI and NSTEMI.Methods:After exclusion, a total of 113 patients who were admitted to our hospital due to ACS and who were referred for undergoing coronary angiography were prospectively included. Their mean age was 63.3±18.5 years, and 80.5% of them were men. In total, 44.2% of the patients had NSTEMI and the remaining had STEMI. Blood samples were drawn at admission to evaluate serum 25(OH)D levels. CAD severity was assessed using the SXscore. Patients were classified as having low (SXscore ≤22) or high (SXscore >22) SXscores. Pearson’s and Spearman’s correlation coefficients were used to examine the relationship between serum 25(OH)D levels and the SXscore.Results:25(OH)D levels were significantly lower in the group with a high SXscore than in the group with a low SXscore (21.0±8.0 vs. 16.7±6.8, p=0.005). Correlation analysis showed a significant correlation between 25(OH)D levels and the SXscore. Multiple linear regression (MLR) analysis was used to determine the significance of the relationship between the SXscore and 25(OH)D, parathyroid hormone, and C-reactive protein levels and eGFR. MLR analysis revealed that only 25(OH)D levels (coefficient beta, -0.217, p=0.029) was significantly associated with the severity of CAD.Conclusion:The present study showed that serum 25(OH)D levels were significantly lower in patients with STEMI/NSTEMI and that low serum 25(OH)D levels were significantly correlated with CAD severity and extent.
