Closed reduction and percutaneous “periarticular” single K-wire fixation for displaced unstable transverse fractures of the proximal phalanx in elderly osteoporotic women: A case series of five patients

Abstract: HighlightsNo literature was found on fractures of the proximal phalanx in osteoporotic women. We present a case series of five patients with 7 fractured fingers. The results were excellent in 5 fingers and good in 2 fingers.

“…The wire is then advanced distally, keeping the proximal interphalangeal joint free (Figure 11). No cast is used after surgery and all finger joints undergo immediate mobilization with impressive results (Al-Qattan, 2011a; Alazzam and Al-Qattan, 2017). I recently named this periarticular K-wire fixation for transverse shaft fractures as the ‘unicortical’ technique because the K-wire passes through one cortex (Figure 11).…”

“…Wie bereits von Alazzam und Mitarb. in einer Fallserie von über 70-jährigen Patienten demonstriert, messen auch wir der frühfunktionellen Nachbehandlung eine besondere Bedeutung bei, um in einem hohen Lebensalter rasch ein gutes funktionelles Ergebnis zu erzielen [21]. Bei gegebener Compliance und Frakturstabilität ist diese deshalb einer mehrwöchigen Immobilisierung vorzuziehen.…”

“…The use of Kirschner (K)-wires has been described for the management of phalangeal neck fractures both in children and adults (Alazzam and Al-Qattan, 2017; Al-Qattan, 2001; Al-Qattan, 2006; Al-Qattan, 2010b; Kang et al., 2005). Different insertion techniques have been described, including antegrade, retrograde, criss-cross, intramedullary, single or multiple, introduced by open or percutaneous techniques.…”
