Joint sparing Kirschner-wire fixation for displaced proximal phalangeal neck fractures: randomized prospective comparative study

Abstract: The aim of this study was to compare two joint sparing Kirschner-wire fixation techniques for displaced proximal phalangeal neck fractures. Forty-six patients with proximal phalangeal neck fractures managed by either percutaneous antegrade flexible intramedullary nailing (Group I) or crossed Kirschner-wires (Group II) were recruited for a randomized prospective comparative study. Clinical and radiological assessment of all patients was done in addition to the Kang scoring system, Disability of Arm, Shoulder an… Show more

“…Goodell, Bauer and Sadek determined that closed reduction without pinning is associated with nonunion and its insufficient in maintaining the reduction in type II phalangeal neck fractures [ 17 , 18 ]. Among our 40 patients, there was no case of fracture nonunion or phalanx avascular necrosis at the last follow-up.…”

“…A minimum of two wires should be placed whenever possible to confer stability. Dual wires placed in a parallel or crossing fashion have demonstrated satisfactory outcomes with few complications (Sadek, 2020). Oblique condylar splits with proximal propagation are amenable to fixation with multiple wires placed slightly divergent to each other.…”

“…13,[21][22][23] It is suggested in the literature to start mobilization exercises as soon as fracture stability can be assured to prevent scarring, adhesion formation, and joint stiffness. [23][24][25][26] The systematic review of Feehan and Bassett 27 (2004) concluded that early motion facilitated a faster recovery of metacarpal fractures, which can lead to a quicker resumption of activities of daily living and an earlier return to work. However, no articles on proximal phalangeal fracture management met the eligibility criteria and due to this, the benefit of early motion in this patient population could not be demonstrated at that time.…”
