The inheritance of the autosomal recessive type of gyrate atrophy of the choroid and retina seems well documented (Botermans, I 972), but the aetiology of the disease is still unknown.Animal experiments and some disturbances of lipid, carbohydrate, and protein metabolism connected with chorio-retinal degenerations in man support the possibility of an enzymatic disorder as an aetiological factor of these diseases (Franceschetti, Fran §ois, and Babel, I963; FranSois, I964).The present author recently diagnosed gyrate atrophy of the choroid and retina in an 8-year-old boy in whom an enlarged lysine-ornithine spot was found on the urine highvoltage electrophorogram in routine examination of urinary amino-acids. Further studies identified that the spot was ornithine. The plasma ornithine concentration was more than ten times higher than normal, although normal amounts of the other plasma aminoacids were present. A constant correlation between hyperornithinaemia and gyrate atrophy of the choroid and retina was then detected in nine other patients (Simell and Takki, I973).The purpose of this paper is to describe the different forms of gyrate atrophy found in a total of fifteen patients and to discuss the correlation of fundus changes with hyperornithinaemia. MaterialAn examination of case histories from I950 to 1972, with clinical diagnosis of any primary chorioretinal degeneration, revealed some thirty cases in which gyrate atrophy was suspected. Among them eleven cases of gyrate atrophy of the choroid and retina at different stages were found. Three additional cases were found among relatives of one patient and one further case was referred to the author. The total number of cases with gyrate atrophy of the choroid and retina described in this paper is thus fifteen, nine of which were included in our previous paper (Simell and Takki, I973). The characteristics of the patients are shown in Table I (overleaf). Among the ten families there were four sisters (Patients 6 and 9-I I) and two pairs of sisters (Patients 3, 5, and I 2-I3) and one of these was the first cousin of another patient (Patient 4).There were twenty intact and ten aphakic eyes, four of which were treated because of open-angle glaucoma. One eye was in postoperative phthisis after a cataract operation (Table II, overleaf).

Fifty-four normal tension glaucoma cases were studied to determine the value of several clinical tests for predicting the progression of the disease. Outflow facility, intraocular pressure (IOP) increase after water drinking, and diurnal changes in IOP were studied. Progression was determined on the basis of changes in visual sensitivity as measured on the Octopus 201. A minimum of four examinations of the central 30 degrees were conducted over a 3- to 7-year period. Progression of visual field defects was seen in 38.5% of eyes that had demonstrated some degree of abnormality in at least one of three clinical tests, while only 10.7% of those eyes that appeared normal on the basis of these tests showed such progression. The difference was significant (p < 0.04). These results suggest that the three clinical tests may be of value in detecting normal tension glaucoma eyes at risk for progression of visual field defects.

ACCORDING to Becker, Pyle, and Drews (1959) the instillation of miotics into glaucomatous eyes may be followed by an increase in the tonographically calculated coefficient of aqueous outflow, together with lowering of intra-ocular pressure. Although there is a well-accepted clinical grading of the various miotics by "strength" from pilocarpine at the lower end to the powerful cholinesterase inhibitors at the upper, our knowledge is still surprisingly scanty regarding exact dose-response relationships. The study here described was set up to discover whether pilocarpine 2 per cent. used three times a day or echothiophate iodide 0 12 per cent. used once at night, gave different tonographic and tonometric values on the same eyes. Materials and MethodThe clinical material consisted of 11 patients (18 eyes) suffering from open-angle (chronic simple) glaucoma. All had been under observation for a minimum period of three months and had been subjected to the usual investigations: general ophthalmological examination, gonioscopy, applanation tonometry, and visual field studies. Only wide-angle cases were admitted and none of the eyes studied had been subjected to any form of ocular surgery.The apparatus used in the study was the Haag Streit 900 slit-lamp with applanation tonometer and the Schwarzer electronic tonometer with recorder. Each patient was treated with pilocarpine 2 per cent. drops three times daily, and was instructed to instil one drop in each eye at 8.00 a.m., 4.00 p.m., and 10.00 p.m., and after a minimum period of one week on this therapy applanation tonometry and tonography were carried out. The time of day at which this examination was made varied from patient to patient.Pilocarpine therapy was then changed to echothiophate (Phospholine) iodide 0-12 per cent., one drop in each eye every night at 10.00 p.m. One week later applanation tonometry and tonography were again carried out in each case at the same time of day as upon the first occasion.Tonography results were recorded in the rigidity-corrected form by the graphic method of Moses and Becker (1958).
