Clinical Significance of VEGF-A and Microvessel Density in Diffuse Large B-Cell Lymphoma and Low-Grade Follicular Lymphoma

Abstract: Angiogenesis is essential for the development, growth and progression of tumors. Although vascular endothelial growth factor (VEGF) is a well-known proangiogenic factor, its impact on lymphoma has not yet been fully clarified. The aim of this study was to evaluate VEGF-A -expression and microvessel density (MVD) in aggressive lymphoma such as diffuse large B-cell lymphoma (DLBCL), in indolent lymphomas such as low-grade follicular lymphoma (FL), and in lymph node reactive follicular hyperplasia (FH). In 80 pro… Show more

“…In contrast to most other B-NHL malignancies in which high levels of pro-angiogenic factors and an increased MVD is associated with an adverse prognosis, high level FL vascularization correlates with a beneficial disease course ( 260 – 262 ). The improved clinical outcome apparently correlated with the increased vascularization, but was surprisingly independent of follicular VEGF-A expression ( 223 , 263 ). Some studies stated a minor vascular remodeling in FL compared to reactive LN or follicular hyperplasia, or even vascular regression constraining the growth of reactive and neoplastic follicles ( 260 , 264 ).…”

“…The right diagnosis in these cases are often post-operative. It is very difficult, especially in adults, to make a distinction between BL and diffuse large B-cell lymphoma: doubtful cases should undergo genetic testing and additional analysis because the two diseases are treated differently [ [20] , [21] , [22] ].…”

“…The main challenge in diagnosing BL is distinction between BL and diffuse large B-cell lymphoma, which is difficult. Distinguishing between these two lymphomas, however, is critical, especially in adults, as the two diseases are treated differently ( 3 , 13 , 14 ). Our second case report serves to stress the problem with distinction between the two types of lymphomas.…”
