Clinical predictors in the natural history of uterine leiomyoma: preliminary study.

Abstract: This study was conducted to examine the correlation between uterine vascularity and natural history of uterine leiomyoma. Seventy women with leiomyoma participated in this study. Measurements of uterine and leiomyoma volume, as well as blood flow characteristics of the main uterine artery and leiomyoma arteries, were made every 3 months for 1 year. Leiomyoma arteries could be detected in 52 (51.5%) of 101 leiomyomas. Leiomyoma volume increased in 24 (46.2%) of 52 leiomyomas with leiomyoma artery. However, the … Show more

“…Studies should attempt to determine the extent to which patient characteristics, ultrasonographic features, and physiologic measures are related to UL progression from clinical diagnosis to surgical therapy, particularly hysterectomy. For example, one study suggested that increased tumor vascularity, as measured by Doppler ultrasonography, is strongly associated with increased tumor growth [224]. Similar studies, augmented by analyses of molecular features of excised UL, may help define predictors of recurrence risk following myomectomy.…”

“…1 Aetiologies of uterine leiomyomas are unknown but several predisposing factors have been identified including age (late reproductive years), African-American ethnicity, nulliparity, obesity and genetics. 2 It is seven times commoner in blacks compared to Caucasians 3 and can grow so large to cause negative impact on obstetric outcome if co-existing with pregnancy.…”

“…Symptoms increase with the size of fibroids (7,9,10). However, few studies have examined the growth of fibroids over time (11)(12)(13), and no study has systematically followed the growth of fibroids in black and white women.…”
