Clinical Pharmacology and Vascular Risk

Abstract: Pharmacological treatment and several drugs of abuse have been associated with ischemic heart disease (IHD) and cerebrovascular diseases (CVD). However, there is a paucity of data on the independent risk of vascular disease (VD) associated with pharmacological treatment and no controlled trials demonstrating a reduction in risk with abstinence.Information about IHD and CVD-related drug abuse is mainly limited to epidemiological studies focused on urban populations. The potential link between some pharmacologic… Show more

“…The overall risk of ischemic stroke is increased nearly sevenfold with drugs of abuse; this is especially true for cocaine (7). For reasons that are not clearly understood, the alkaloidal form of the drug is associated with an equal frequency of ischemic and hemorrhagic events, whereas the hydrochloride form is associated with much higher rates of hemorrhage (5).…”

“…However, compared to antipsychotic medications, Gabapentin has shown to be safer in use for patients with cerebrovascular accidents. As a matter of fact, Gabapentin is the only anticonvulsant medication studied in the management of patients with stroke [6].…”

“…The majority of cocaine-using patients are young males who are current or former smokers and less likely to have a history of coronary artery disease, hypertension, dyslipidemia, diabetes mellitus, or congestive heart failure when compared to non-cocaine users. 1,11 These patients are at low risk for cardiac events, as MI was rare and myocardial ischemia uncommon, as demonstrated by SPECT MPI (only 6% of the cocaine-using patients had abnormal resting perfusion on MPI). None of these patients while in the hospital had cardiac arrest or congestive heart failure.…”
