The aim of the present study was to compare the restorative time for filling cavities in posterior teeth using bulk‐fill and conventional resin composites through a systematic review and meta‐analysis. A search for clinical trials and laboratory studies was performed in PubMed/MEDLINE, Scopus, the Latin American and Caribbean Health Sciences Literature database, the Brazilian Library in Dentistry, Cochrane Library, Clinical Trials, and ReBEC (Registro Brasileiro de Ensaios Clínicos) databases without publication year or language restriction. Two reviewers identified eligible studies according to the inclusion criteria: bulk‐fill compared to conventional resin in class I or II, and the restorative time as an outcome. A meta‐analysis of the restorative time mean difference between composites was performed (inverse variance method, random effects model; Z‐test, P ≤ .05). From the 662 eligible studies, 133 were selected for full‐text analysis; three were included in the systematic review and in the meta‐analysis. Overall, the restorative time was lower when bulk‐fill was used (P = .0007, Z = 3.37), as the subgroup full‐body bulk‐fill (P < .00001, Z = 21.00). There was no difference in restorative time between flowable bulk‐fill and conventional resins (P = .08, Z = 1.76). Moderate‐to‐substantial heterogeneity was detected. Full‐body bulk‐fill composites decrease the restorative time in posterior teeth compared to conventional resins. Full‐body bulk‐fill resins require a shorter restorative time to perform restorations in posterior teeth than conventional resins, but the same is not valid for flowable bulk‐fill resin composites.

Objectives Microleakage in direct dental restorations is a primary causal factor in the restoration's failure. The aim of this study was to evaluate whether the technique for occlusal layering of the composite resin (the use of brush adaptation, the use of magnification, cusp build‐up, stamp technique) has any effect on microleakage of direct restorations in occlusal cavities. Materials and Methods One hundred extracted human molars were restored using five restoration techniques (Packable Bulk technique, Occlusal Stamp technique, Successive Cusp Build‐up technique, Successive Cusp Build‐up technique + Brush adaptation, Successive Cusp Build‐up technique + brush adaptation + Dental Operative Microscope magnification). The teeth were subjected to thermal aging for 800 cycles at 5°C and 55°C, infiltrated with basic fuchsin dye for 24 h, and then sectioned buccolingually in the middle of the crown. Infiltration was measured in four areas of the tooth section by five different observers and then given a score from 1 to 3, proportional to infiltration depth. Results The lowest mean scores for infiltration (meaning less infiltration observed) were present in Group A (1.41 ± 0.878) and Group C (1.46 ± 0.679), while Group D showed the highest infiltration scores (1.75 ± 0.853). When comparing the groups for differences, no statistically significant difference in infiltration was found between any technique p < .586. Conclusion The techniques examined for placing the occlusal layer of composite in direct restorations do not differ significantly in terms of marginal infiltration, although a slight improvement was found when using the bulk technique and the successive cusp build‐up.

The quality of the micromechanical retention between the enamel prisms exposed through acid etching and adhesive system can be influenced by the relationship between the angle of the exposed enamel prisms and the external surface/restorative material. Therefore, the aim of the present study was to determine the influence of the enamel cavosurface beveling in different cavity sizes on the angle relation between enamel prisms and the restorative material. For this research, 30 human permanent molars were used and divided into three groups according the isthmus aperture of class I cavities (1/4, 1/3, and 1/2 of the intercuspal distance). Cavosurface angle beveling was performed in each tooth buccal or palatal/lingual side, and a resin composite restoration was placed. Each restored tooth was sectioned in buccal/palatal direction followed by analysis in scanning electron microscope. Means of the angles observed between enamel prisms and the restorative material were registered and statistically analyzed by Kruskal‐Wallis test. Results showed higher mean angle values for beveled groups when comparing with the values observed in the unbeveled margins (p < .05) regardless of the isthmus aperture, being 1/4 (beveled = 48.36°; unbeveled = 20.71°), 1/3 (beveled = 39.75°; unbeveled = 29.15°), and 1/2 (beveled = 37.02°; unbeveled = 26.34°). It was also observed that in some unbeveled cavities, there were the presence of laterally exposed enamel prisms (0°), mainly in more conservative cavities. It was concluded that the presence of higher angles values occurred in the samples with beveled margins, without presence of laterally exposed enamel prisms.
