Clinical outcomes after intravascular ultrasound and fractional flow reserve assessment of intermediate coronary lesions. Propensity score matching of large cohorts from two institutions with a differential approach

Abstract: IVUS and FFR may be safely used to defer revascularisation of intermediate lesions. IVUS induces a higher degree of revascularisation but much lower than previously reported and does not affect the clinical outcome.

“…Thus, we have conducted a similar analysis involving the use of this new parameter: acute drop Pd/Pa‐NTG. In our multicenter study the average FFR measurements and proportion of significant values are in accordance with those reported in previous large experiences from real practice . An excellent correlation has been found between acute drop Pd/Pa‐NTG and FFR with a cut‐off value of >0.88 associated with very high NPV (96.2% for FFR < 0.8 and 100% for FFR < 0.75).…”

“…The PS Matching program performs all analyses in R through the SPSS-R Plugin (Thoemmes F, 2011) – an SPSS R menu for propensity score matching. [ 10 , 11 ]. Variables for which the unadjusted p value was < 0.10 were identified in multivariate logistic regression analysis as potential risk markers and included in the full model.…”

“…Alltogether this suggests that most of the vulnerable intermediate lesions are already treated based on FFR measurements anyway, but it should be noted that a high consternation of necrotic core was also observed in FFR negative lesions [14]. Clinical observations presented similar clinical outcomes for the coronary lesion discrimination by IVUS or FFR [16], despite the limited utility of IVUS measurements to guide intervention [17]. Similar clinical outcomes for IVUS and FFR guided interventions suggest that some of IVUS guided intervention were preformed in FFR-negative vulnerable lesions, which brought benefit to patients.…”
