Cyclical mastalgia is a common complaint, with a potentially important relationship to breast cancer risk. In the last decade, case-control studies have reported that cyclical mastalgia could be considered as an independent risk factor for breast cancer. The subjectivity of a retrospectively collected symptom questioned the validity of this finding. We have examined the association between cyclical mastalgia and breast cancer risk in the French cohort study of women with benign breast disease diagnosed in two breast clinics between 1976 and 1979 and followed-up until 1997. The present study was restricted to the women free of any hormonal treatment (n = 247). The mean follow-up was 16 F 5 years, and a total of 22 breast cancers occurred during the follow-up. Using a Cox model with duration of cyclical mastalgia as a time-varying variable, the adjusted relative risk of breast cancer increased with the duration of cyclical mastalgia (P = 0.006). The corresponding relative risk for 37 months of cyclical mastalgia was 5.31 (95% confidence interval, 1.92-14.72). We show here that the conclusion still holds when the symptom cyclical mastalgia was collected prospectively in a cohort study, bringing additional evidence that cyclical mastalgia may represent an independent and useful clinical marker of increased breast cancer risk. It might be a confounding factor when assessing the effects of hormonal treatments on breast cancer risk such as hormonal replacement therapy or oral contraceptives. In order to evaluate the safety and efficacy of Borago officinalis (900 mg borage oil capsules) in the treatment of patients presenting with cyclic mastalgia, 91 subjects were included in the study. Efficacy assessments were performed based on data obtained prior to the start of treatment (Pretreatment), and after each menstrual cycle (Assessment 2-following 45 days of treatment; and Assessment 3-at the end of the 90-day treatment period). Primary efficacy measures considered the results of the Mastalgia Questionnaire, including a 100 mm visual analog pain scale assessing mean, most intense mastalgia severity, and impact on work, sleep, and sexual activity. Safety and tolerability measures included any changes in vital signs and physical exam in relation to pretreatment, any changes in clinical laboratory exams in relation to pretreatment, and the occurrence of adverse events after the first dose of study medication. The VAS scores of the mean mastalgia and most severe mastalgia both showed statistically significant (p < 0.0001) reductions from Pretreatment to Assessment 3. Mean mastalgia scores improved among 92.3% of the treated patients, while most severe mastalgia scores improved among 93.4% of patients. There were sta-* Corresponding author. C. R. B. Gama et al. 364tistically significant improvements in the assessments of mastalgia impact on work (χ 2 = 28.24; gl = 4; p < 0.0001), sleep (χ 2 = 14.29; gl = 4; p = 0.0006), and sexual activity (χ 2 = 16.11; gl = 4; p = 0.0029) during the treatment period. The results of this study indicate a significant improvement in the mastalgia of the treated patients together with an improvement in the quality of life parameters evaluated. In terms of safety, the tolerability of the treatment was good, with the presence of some adverse events, all of which had been previously described with use of the Borago officinalis extract. No serious side effects were reported, and the events that did occur were transitory. Based on the results of this study, we concluded that the Borago officinalis extract was safe and effective in the treatment of cyclic mastalgia among the treated patients.

Objective: Mastalgia is a common complaint encountered in polyclinics. The causative pathologies of mastalgia are either because of the breast or other regions. We aimed to share the causes of mastalgia which we detected in the polyclinics. Materials and Methods:The retrospective evaluation of the files of 1150 patients with mastalgia out of 3200 patients who had applied at our breast polyclinics between June 2011 and August 2012 was carried out. When evaluation of 620 patients with mastalgia had been made, exclusion criteria were the presence of concomitant complaints such as mass, discharge, retracted nipple. Patients with cyclic mastalgia complaints were also excluded from the study. Pure complaints of mastalgia in 530 patients were evaluated for organic pathologies in regard to anemnesis, physical examination, radiological and laboratory findings.Results: When 530 patients with pure mastalgia were evaluated, breast pathologies were detected in 115 patients. Fibrocystic mastopathy in 67, fibroadenoma in 18, cyst in 19, abscess in 3, granulomatous mastitis in 2, breast cancer in 2 and high prolactin levels in 4 patients were detected. The remaining 20 patients were sent to radiology guided biopsies and were excluded from the records. A total of 35 patients with mastalgia had had psychiatric drug usage either currently or in the past. No breast pathology was detected in 395 of 530 patients and 53 of those 395 patients were sent to orthopedic consultation in whom 36 were found to suffer from cervical or shoulder pathologies. Conclusion:When patients with mastalgia were evaluated, it was detected that both breast pathologies and nonbreast problems concerning the breast were the reasons for admission to polyclinic because of the pain. Reasons for pain with no breast origin should also be investigated in patients with complaints of pure mastalgia and without radiological evidence of breast disease.Key words: Mastalgia, polyclinic, mastodinea ÖZET Amaç: Çalışmamızda; mastalji şikayeti bulunan hastalarda tespit edebildiğimiz nedenleri paylaşmayı amaçladık. Yöntem ve Gereçler:2011 Haziran-2012 Ağustos arasında meme polikliniğine başvuran 3200 hastadan, mastalji yakınması olan 1150 hasta dosyası retrospektif olarak incelendi. Mastalji yanında ek şikayeti bulunan (kitle, akıntı, meme başı çekintisi gibi) 620 hasta çalışma dışı bırakıldı. Siklik mastalji şikayeti olan hastalar değerlendirmeye alınmadı.Bulgular: Pür mastalji şikayeti olan 530 hasta incelendiğinde, 115 hastada meme patolojisi saptandı. Seride 67 hastada fibrokistik mastopati, 18 hastada fibroadenom, 19 hastada kist, 3 hasta da abse, 2 hastada granulamatöz mastit, 2 hastada meme kanseri saptandı. Ayrıca 4 hastada ise prolaktin yüksekliği tespit edildi. Hastaların 395 tanesinde tetkikler neticesinde memeye ait herhangi bir patoloji saptanmadı. Bu hastalar tekrar sorgulandı ve bazılarında meme dışında boyun, omuz, kol, sırt bölgelerinde ağrı tariflediği görüldü.
