Clinical implications of a feeling of incomplete emptying with little post‐void residue in men with lower urinary tract symptoms

Abstract: A feeling of incomplete emptying despite little PVR was frequently observed and, notably, this was significantly associated with worsening of both voiding and storage symptoms, even after adjusting for relevant variables.

“…Indeed, the IPSS, the most popular symptom assessment tool, includes a question about a feeling of incomplete feeling but not post‐micturition dribble as a post‐micturition symptom. However, population‐based studies in which post‐micturition symptoms were specifically assessed showed that these symptoms are frequently observed along with storage and voiding symptoms (in 11.8%‐64% of men, and 8.5%‐32.5% of women),and could be an additional burden on those with voiding and/or storage symptoms . In particular, those with a feeling of incomplete emptying were most bothered by their health problems .…”

“…To the best of our knowledge, there have been only 2 studies of the association between LUTS and little PVR, although neither study addressed the treatment outcome. In one study, Lee et al demonstrated that a feeling of incomplete emptying was poorly correlated with little PVR (≤20 mL) and that the severity of this feeling was significantly associated with the subscores of both voiding and storage symptoms on the IPSS in men. In another study, Huang et al examined the prevalence of urinary symptoms according to PVR category (<50, 50–99, 100–199, and ≥200 mL) in women.…”

“…The number and severity of symptoms can easily be assessed semiquantitatively using the International Prostate Symptom Score. 12,13 A voiding diary, which includes frequency, volume voided, and fluid consumption during several 24-hour periods, is particularly useful in identifying nocturnal polyuria or excessive fluid intake. 14 Nocturnal polyuria is defined as voiding more than 33% of the 24-hour urine production at night, and its presence suggests a pathophysiologic cause outside the lower urinary tract.…”

“…The severity of the feeling of incomplete bladder emptying has been associated with the degree of LUTS and, notably, with worsening of both voiding and storage symptoms, although it improves simultaneously along with other LUTS with oral pharmacotherapy.…”

“…Although the feeling of incomplete bladder emptying can be accounted for within the clinical concept of bladder outlet obstruction (BOO) with high PVR, the sensation of not being able to empty the bladder in the absence of significant PVR is subjective, of unclear etiology, and the subject of ongoing clinical studies …”
