Introduction. Bladder and bowel dysfunction describes a large spectrum of lower urinary tract symptoms along with fecal elimination issues. The aim of the study was to analyze the effects of biofeedback treatment in children with lower urinary tract symptoms. Material and Methods. A prospective study analyzed the effects of biofeedback treatment conducted in children with lower urinary tract symptoms. Questionnaires and voiding diaries were collected prior to the treatment. The patients were followed for two weeks on daily basis. After the completion of the treatment, the data from voiding diaries and questionnaires were analyzed. Results. A total of 18 children were referred for biofeedback treatment. Seven patients had an overactive bladder. seven had isolated dysfunctional voiding, and in the third group three had difficulties starting to void and one had daily incontinence with dysfunctional voiding. A total of 14 patients presented with improvement of symptoms. The analyzed data showed no measurable improvement in one patient, even though he reported a personal feeling of improvement. Three patients without positive effects of the therapy were immature and non-cooperative. In six out of seven patients with overactive bladder with urine leakage, the symptom disappeared by the end of the treatment. Conclusion. Biofeedback is a very useful tool in the treatment of lower urinary tract symptoms in pediatric population. Although the main indication for initiating this therapy is dysfunctional voiding, the study showed an improvement of symptoms in patients with overactive bladder as well.

PURPOSE:To elucidate six diagnostic concepts related to urinary incontinence (UI). METHODS: Eight out of nine steps of the simultaneous concept analysis (SCA) method were used to elucidate six diagnostic concepts related to UI proposed by NANDA International. FINDINGS: Individual concepts of each diagnosis were elucidated, including antecedents, outcomes, associated conditions, and risk populations, and the interrelations among these concepts were identified, comparing them to those already present in the current NANDA-I taxonomy. The overflow UI was excluded, and were included mixed and transient UI diagnoses. CONCLUSIONS: SCA enabled identifying similar characteristics and factors exclusive to the revised nursing diagnoses. IMPLICATIONS FOR NURSING PRACTICE:To support for clinical reasoning to identify properly the types of UI.OBJETIVO: Elucidar seis conceitos diagnósticos relacionados à incontinência urinária. MÉTODOS: Oito das nove etapas do método de análise simultânea de conceitos foram utilizadas para elucidar seis conceitos diagnósticos relacionados à incontinência urinária propostos pela NANDA International. RESULTADOS: Conceitos individuais de cada diagnóstico foram elucidados, incluindo antecedentes, consequentes, condições associadas e populações em risco, e as inter-relações entre esses conceitos foram identificadas, comparando-as com aquelas já presentes na atual taxonomia da NANDA-I. A incontinência urinária por transbordamento foi excluída e foram incluídos os diagnósticos de incontinência urinária mista e incontinência urinária transitória. CONCLUSÕES: A análise simultânea de conceitos permitiu identificar características e fatores semelhantes exclusivos aos diagnósticos de enfermagem revisados. IMPLICAÇÕES PARA A PRÁTICA DE ENFERMAGEM: Apoiar o raciocínio clínico para identificar adequadamente os tipos de incontinência urinária.

Introduction:Neurogenic bladder patients are at long-term risk of secondary upper urinary tract damage. Symptoms are unreliable and follow-up urodynamics is the only method of ascertaining safety of bladder pressures. This review examines the recommendations, shortcomings and utilization of existing guidelines. The evidence with regard to follow-up urodynamics in different settings relevant to neurogenic bladder is evaluated and an algorithm is proposed.Methods:A pubmed search was conducted for studies on follow-up urodynamics in patients with neurogenic bladder. Additional search was made of secondary sources including reviews and guidelines.Results:The need for follow-up urodynamics should be considered in all patients undergoing an initial assessment and weighed against the risks. Existing guidelines, while unanimous in their recommendation of its utilization, give scant details regarding its incorporation in clinical management. Follow-up urodynamics can document efficacy and identify the need for escalation of therapy in patients on intermittent catheterization and antimuscarinics. Patients with spinal injury, spinal dysraphism and anorectal malformations are at higher risk for upper tract damage. Follow-up urodynamics can help identify patients suitable for intravesical botulinum and mark those destined for failure. Patients undergoing augmentation cystoplasty may be candidates for less aggressive urodynamic follow-up.Conclusions:Neurogenic bladder is managed by a broad cross-section of physicians. Clear recommendations and a management algorithm are important for improving patient care. Follow-up urodynamics can identify patients at risk, prevent renal dysfunction and improve the quality of life. There is an urgent need for more evidence on this important subject.
