Clinical features and management of carboplatin-related hypersensitivity reactions in pediatric low-grade glioma

Abstract: This is the largest single-center study conducted to investigate the frequency and the management strategies in patients with CHR who were treated with the same chemotherapy protocol for LGG. Premedication and desensitization were the preferred modifications in case of CHR. Overall, the success rate for carboplatin continuation is high in comparison to previous studies. Carboplatin can be continued successfully in many cases with CHR if reactions are managed in a timely fashion.

“…As a summary, only four of 19 (21 %) patients (four of 50 (8 %) LGG patients) were given corticosteroids. In our manuscript [2], we also recommend antihistamines±desensitization protocol at first CHR episode and try to avoid routine administration of corticosteroids.…”

“…
Dear Editor, We thank Dr Lafay-Cousin for her interest in our study [1,2].Our study population was composed of 50 low-grade glioma (LGG) patients and 20 of them developed carboplatin-related hypersensitivity reactions (CHR). Four out of 19 re-exposed patients at first episode and two out of eight patients at second episode were started on corticosteroids as part of their modification regimen.

“…Surgical excision of the tumor is recommended as the primary treatment. However, because of its infiltration into normal brain tissue and the specificity of its growing locations (Ong et al, 2009), the tumor usually cannot be removed completely and thus adjuvant radiotherapy and/or chemotherapy have to be employed so as to kill the rest of the tumor (Genc et al, 2011). Treatment of gliomas is complicated by the tumors' high resistance to chemotherapy, poor penetration of blood brain barrier (BBB), and damaging effects of chemotherapy and radiation to normal neural tissue (Lefranc et al, 2006).…”

“…6 We were not able to detect any significant differences at a higher radiation dose, possibly due to the insensitivity of our method. 4 leading to treatment withdrawal and the use of second-line drugs. Additionally, carboplatin administered in association with vincristine has been found to be a risk factor for adverse events, 5 raising the incidence to 68% of children treated.…”
