Effective treatment of epilepsy depends on medication compliance across a lifetime, and studies indicate that drug tolerability is a significant limiting factor in medication maintenance. Available antiepileptic drugs (AEDs) have the potential to exert detrimental effects on cognitive function and therefore compromise patient wellbeing. On the other hand, some agents may serve to enhance cognitive function. In this review paper, we highlight the range of effects on cognition linked to a variety of newer and older AEDs, encompassing key alterations in both specific executive abilities and broader neuropsychological functions. Importantly, the data reviewed suggest that the effects exerted by an AED could vary depending on both patient characteristics and drug-related variables. However, there are considerable difficulties in evaluating the available evidence. Many studies have failed to investigate the influence of patient and treatment variables on cognitive functioning. Other difficulties include variation across studies in relation to design, treatment group and assessment tools, poor reporting of methodology and poor specification of the cognitive abilities assessed. Focused and rigorous experimental designs including a range of cognitive measures assessing more precisely defined abilities are needed to fill the gaps in our knowledge and follow up reported patterns in the literature. Longitudinal studies are needed to improve our understanding of the influence of factors such as age, tolerance and the stability of cognitive effects. Future trials comparing the effects of commonly prescribed agents across patient subgroups will offer critical insight into the role of patient characteristics in determining the cognitive impact of particular AEDs.

Abstract. The impact of epilepsy on Quality of Life (QoL) is well-documented. The ability of epileptic seizures to alter the conscious states of patients is also well established. Although there is much research on the QoL of people with epilepsy, few researchers have looked specifically at the effect of sudden, unanticipated alterations of consciousness on QoL. This lack of systematic studies of consciousness alterations and QoL in epilepsy limits our ability to shed light on this interrelation. In this article, with these limitations in mind, we focus on studies of newer AEDs. We review the evidence as to whether a significant reduction (typically more than 50%) in seizures that induce alterations of consciousness, as a result of switching to one of the newer AEDs, leads to improvements in QoL. We draw on this literature to speculate on the relationship between ictal consciousness alterations and poor QoL in epilepsy, to identify contributory and confounding factors and to highlight implications for future research. We suggest that an understanding of how factors associated with consciousness impairment affect QoL could help the treatment and management of these patients.

В последнее время появляются работы от исследователей из разных стран мира, раскрывающие дополнительные эффекты левети-рацетама, позволяющие использовать его не только как противоэпилептический, но и как нейропротективный и антигипералгезивный препарат. Обзору таких данных посвящена данная публикация.Леветирацетам был синтезирован в начале 1980-х го-дов. Молекула принадлежит к семейству пирролидонов, классу препаратов, обладающих широким спектром действия. Начальные фармакологические исследования леветирацетама показали его способность влиять на хо-линергическую нейротрансмиссию. Леветирацетам был одобрен в США в 1999 году Управлением по санитарно-му надзору за качеством пищевых продуктов и медика-ментов, а в 2000 году он был одобрен в Европе для ис-пользования у взрослых пациентов с миоклоническими судорогами, ювенильной миоклонической эпилепсией и при первичных генерализованных тонико-клониче-ских судорогах [30]. В 2012 году FDA одобрило левети-рацетам для использования в качестве вспомогатель-ной терапии парциальных припадков у детей в возрасте 1 месяца и старше [8].Леветирацетам обладает хорошим фармакокине-тическим профилем и используется в разнообразных клинически сложных ситуациях. Леветирацетам до-казал свою эффективность в профилактике различных форм эпилепсии, применялся в качестве монотерапии [2, 27] и дополнительного лечения для предотвращения ранних посттравматических припадков [1, 10, 13, 33]. В последнее время появились данные об эффективно-сти и безопасности леветирацетама при лечении при-ступов эпилепсии у новорожденных [31]. Кроме того, исследования показали, что леветирацетам обладает значительными нейропротективными свойствами при купировании неэпилептических [30] и эпилептических приступов [18,25]. Экспериментальные данные свиде-тельствуют о том, что леветирацетам оказывает анти-гипералгическое действие при ноцицептивной [20, 26] и нейропатической боли [9,23]. Механизм действия леветирацетамаНесмотря на множество исследований, посвященных изучению эффективности леветирацетама в качестве ней-ропротектора, антиконвульсанта и анальгетика, фармако-динамика этого препарата до конца не выяснена.
