Clinical effects of intensive insulin therapy treating traumatic shock combined with multiple organ dysfunction syndrome

Abstract: The therapeutic effects of intensive insulin therapy in treatment of traumatic shock combined with multiple organ dysfunction syndrome (MODS) were investigated. A total of 114 patients with traumatic shock combined with MODS were randomly divided into two groups: control group (n=56) treated with conventional therapy, and intensive insulin therapy group (n=58) treated with conventional therapy plus continuous insulin pumping to control the blood glucose level at range of 4.4-6.1 mmol/L. “…A previous study, conducted by the authors of the present study, has demonstrated that IIT improves the prognosis of patients with injury and with pyemia and decreases their fatality rates (26). Another study has reported that LMWH noticeably improves the prognosis of hyperlipidemic pancreatitis and that the underlying mechanisms may be correlated with the improvement of microcirculation, catabolism and anabolism, and the promotion of anti-inflammatory reactions (27).…”

“…Clinicians in the People’s Republic of China have attributed gastrointestinal dysfunction to MODS since 1995. 12 In their scoring system, abdominal bloating or decreased bowel sound is counted as 1 point, a high amount of abdominal bloating or almost disappearing bowel sound is counted as 2 points, and paralytic ileus or stress ulcer bleeding is counted as 3 points. As this scores system can quantify the intestinal dysfunction, it is usually applied to evaluate gastrointestinal function of critical patients in clinical practice and trials by Chinese clinicians.…”
