La halitosis ha sido definida como un olor ofensivo y fétido en el aliento producido por factores como: mala higiene oral, infecciones dentales u orales y/o la ingestión de ciertos alimentos. El objetivo de esta revisión es evaluar el estado actual del diagnóstico, clasificación y tratamiento de la halitosis, y brindar estrategias y herramientas correctas para abordar una problemática que es tan común en la consulta odontológica. La prevalencia de la halitosis es alta y varía dependiendo de la población estudiada y el método de diagnostico usado (escalas subjetivas, mediciones organolépticas, halimeter, cromatografía de gas, test BANA ). Existen 3 tipos de halitosis: La halitosis genuina (fisiológica y patológica), la pseudohalitosis y la halitofobia. La etiología es multifactorial y existe un acuerdo en que principalmente ocurre por el metabolismo de las bacterias. La producción de compuestos volátiles y no volátiles es común en los humanos y está influenciada por factores genéticos, dieta, estrés, enfermedades y/o desordenes presentes en los individuos. Los dos tratamientos más utilizados y tradicionales para el control de la halitosis son el control mecánico de la lengua saburral y el uso de agentes antimicrobianos. La halitosis afecta una gran parte de la población, puede crear desventajas sociales y sicológicas, y aunque es de origen multifactorial su principal causa se encuentra en la cavidad oral, principalmente por factores como la placa bacteriana, gingivitis y periodontitis. El objetivo de este artículo es revisar las herramientas actuales para el diagnóstico y tratamiento de la halitosis.Palabras clave: Halitosis, Enfermedades periodontales, Biopelícula, Pruebas de halitosis. AbstractHalitosis has been defined as an offensive and fetid breath odor caused by factors such as poor oral hygiene, dental or mouth infections and / or ingestion of certain foods. The aim of this review is to assess the current state of diagnosis, classification and treatment of halitosis, providing the right strategies and tools to address a problem that is so common in dental practice. Halitosis prevalence is high and varies depending on the population studied and the diagnostic method used (subjective scales, organoleptic measurements, halymeter, gas chromatography, BANA test). There are 3 types of halitosis: Genuine halitosis (physiological and pathological), the pseudohalitosis and halitophobia. The etiology is multifactorial and there is an agreement about the cause primarily by bacterial metabolism. ResumoObjetivo: Devido ao aumento do número de idosos que vivem em instituições de longa permanência são essenciais projetos de ações para melhorar a saúde bucal. Assim, o objetivo deste estudo foi avaliar o impacto de um programa educacional para os cuidadores de idosos institucionalizados por meio da avaliação da higiene bucal. Material e método: O programa de educação consistiu em palestras para cuidadores sobre saúde bucal que foram realizadas uma vez por mês. Foram avaliados 40 idosos institucionalizados dependentes funcionalmente e 14 cuidadores. Os hábitos de higiene, o índice de placa e o revestimento/descoloração da língua dos idosos foram avaliados antes do programa educacional (T0) e após 6 (T1), 12 (T2), 18 (T3) e 24 meses (T4). Os cuidadores responderam perguntas sobre seus conhecimentos, suas dúvidas e sobre a implementação de cuidados bucais (T0 a T4). Resultado: Após análise dos dados (Friedman, Qui-quadrado e Spearman, α = 0,05), observou-se melhora gradual na higiene oral dos idosos, com maior freqüência de escovação (p = 0,0005), alteração do tipo de escova (p = 0,0065) e redução do índice de placa (p <0,05) e revestimento da língua (p <0,05). Os cuidadores mostraram uma melhoria acentuada no seu conhecimento sobre saúde bucal. Conclusão: Concluiu-se que o programa educacional para cuidadores teve um impacto positivo na saúde bucal dos idosos institucionalizados observado pelo aumento na eficácia dos parâmetros de higiene bucal, como índice de placa e revestimento da língua, contribuindo para o ganho de conhecimento em higiene pelos cuidadores.Descritores: Cuidadores; assistência odontológica para idosos; educação; instituição de longa permanência para idosos; assistência à idosos. AbstractObjective: Due increased number of elders living in long-term care institutions, actions designs to improve their oral health are essential. Thus, the aim of this study was to evaluate the impact of an educational program for the caregivers through the assessment of the oral hygiene of institutionalized elders. Material and method: The education program consisted in lectures to caregivers about oral health that were performed once a month. The subjects were 40 functionally dependent institutionalized elders and 14 caregivers. Hygiene habits, plaque index, and tongue coating/discoloration of the elders were measured before the educational program (baseline-T0) and after 6 (T1), 12 (T2), 18 (T3) and 24 months (T4). Caregivers answered questions about their knowledge, doubts and implementation of dental care (T0 to T4). Oral malodor, also called halitosis or bad breath, is universally experienced condition that has a variety of etiologic factors. It is extremely common and majority of adult population have had it at some point in time. Halitosis may be physiological, pathological, and sometimes psychological also. Although numerous non-oral sites and many different causes have been correlated to bad breath, an estimated 80 percent to 90 percent of all bad breath odors originate from the mouth, and bacteria are directly responsible for most of the offensive gases. Specific groups of bacteria have been identified with the production of oral malodor, in particular, gram-negative, anaerobic bacteria. The unpleasant smell of breath mainly originates from volatile sulfide compounds (VSCs), especially hydrogen sulfide (H 2 S), methylmercaptan (CH3SH), and dimethylsulfide [(CH 3 ) 2 S] result from the proteolytic degradation of peptides present in saliva, shed epithelium, food debris and gingival crevicular fluid (GCF). This article reviews the etiology, diagnosis, and treatment of oral malodor and gives a brief description of Halitosis Associated Life-quality Test (HALT) questionnaire, a specific 20-item quality-of-life measure for halitosis. The tongue plaque coatingResearch suggests that the tongue is the primary site in the production of oral malodor. The dorsoposterior surface of the tongue has been identified as the principal location for the intraoral generation of VSCs [12]. A variety of index systems for tongue coating has been developed over the years. Miyazaki et al divides the tongue into three sections and the presence or absence of tongue coating is registered as follows: Score 0 = none visible, Score 1 = less than one third of tongue dorsum is covered, Score 2 = between one and two thirds, Score 3 = more than two thirds [13]. Winkel et al divides the tongue into six sections, three in the posterior and three in the anterior part of the tongue. Each sextant is categorized as: Score 0 = no coating present, Score 1 = presence of a light coating, Score 2 = presence of a distinct coating. The resulting Winkel tongue coating index (WTCI) is obtained by adding all six scores (Figure 1) [14].
