Clinical course of light-chain smouldering multiple myeloma (idiopathic Bence Jones proteinuria): a retrospective cohort study

Abstract: Summary Background Bence Jones proteinuria is a disorder that is defined by the excretion of monoclonal light-chain protein. About 15–20% of patients with multiple myeloma secrete monoclonal light chains only, without expression of the normal immunoglobulin heavy chain, which constitutes light-chain multiple myeloma. The definition, prevalence, and progression of these premalignant phases of light-chain multiple myeloma have not been fully characterised. We aimed to identify a subset of patients with idiopath… Show more

“…Risk of progression to symptomatic MM within 2 years was 58-61% [57,58]. Risk progression in light-chain SMM is approximately 5% per year during the first 5 years and decreases thereafter [23]. This risk of progression is lower when compared to conventional SMM, but higher when compared to light-chain MGUS.…”

“…However, in idiopathic Bence Jones proteinuria serum FLC ratio (<0.01 or >100) is associated with a higher risk of progression to MM or AL amyloidosis. 36 Other prognostic factors in idiopathic Bence Jones proteinuria include size of urine Mprotein, BM plasma cell percentage, serum creatinine, and reduction of all 3 uninvolved immunoglobulins. …”
