Background Coronary artery fistula is defined as a connection between one or more coronary artery and a heart chamber. aims The aim of the study was to determine the overall incidence of coronary artery fistulas in Polish patients undergoing diagnostic coronary artery angiography as well as the frequency of particular origin and draining sites. methods The data were obtained from the Polish National Registry of Invasive Cardiology Procedures among patients hospitalized between January 1, 2014 and December 31, 2016 in invasive cardiology departments in Poland. results The study included 298 558 patients. A coronary artery fistula was present in 261 patients (0.087%). There were 131 women (50.19%) and 130 men (49.81%). The most frequent origin of a coronary artery fistula was the left anterior descending artery (167 cases, 59.22%). The pulmonary artery was the most frequent drainage site for fistulas originating from the left anterior descending artery and the right coronary artery (84 cases, 50.30% and 25 cases, 31.65%, respectively). Fistulas originating from the circumflex artery most frequently drained into another artery (10 cases, 27.78%). conclusions The left anterior descending coronary artery was the origin site for more than half of all detected coronary artery fistulas. The pulmonary artery was the most frequent origin and drainage site for coronary artery fistulas.

Primary hyperaldosteronism (PH) is a frequently overlooked cause of secondary hypertension mostly due to an aldosterone-producing adrenal adenoma. We report the case of a 48-year-old female who presented with recurrent episodes of polymorphic ventricular tachycardia before an aldosterone-producing adrenal adenoma was diagnosed. Although her past medical history was remarkable for poorly-controlled hypertension since age 30, she had never been examined for secondary causes. The patient underwent adrenalectomy resulting in stabilisation of her clinical condition, although residual hypertension remained. Causes of atypical presentation and diagnostic pitfalls are discussed.

This publication presents the clinical observation in a paediatric patient with haemangioma of the left stomach. The effectiveness of surgical treatment of this disease is also noted. We assess the challenges in diagnosis and the choice of treatment of this pathology in children and the effectiveness of surgical treatment disease. Primary cardiac tumours are rare; vascular tumours and haemangiomas are rarer. The clinical picture of heart haemangioma is non-specific and varies as per its location and size. Echocardiography is the main diagnostic method for this disease. However, in most cases of preoperative diagnostic examination, the pre-surgery diagnosis is not confirmed. The final diagnosis of capillary haemangioma is established following immunohistochemical staining of the surgical material. Thus far, owing to the rare occurrence, a generally accepted tactic for the treatment of such patients with vascular heart tumours has not been developed. This clinical case describes our experience of treating a rare pathology in children with unusual localisation as well as the rapid successful surgical removal of the tumour without complications and with a favourable post-surgery recovery period.Received 14 May 2020.
