Clinical characteristics of acyclovir-resistant herpetic keratitis and experimental studies of isolates

Abstract: Topical immune depression may induce ACV-resistant HSV-1 infection in the cornea, with a prolonged course in association with ocular complications. The prolonged infectious course of the viral isolates in the animal study partially supported the clinical demonstrations in the patient. The existence of latent infection by one ACV-resistant HSV-1 in its animals may indicate the possibility of its recurrence. Trifluridine may be an alternative choice for treating corneal epithelial lesions caused by ACV-resistant… Show more

“…Less frequent are mutations in the viral DNA polymerase gene that may lead to cross-resistance to FOS [7,8]. In contrast to the extensive study of genital herpes and herpes labialis, large surveys of the incidence of ACV resistance in patients with HK are lacking [4 -8]; thus far, there have been only a few anecdotal case reports of ACV R corneal HSV-1 in patients with HK [4,[9][10][11][12]. In the present study, we determine the prevalence and molecular characteristics of ACV R corneal HSV-1 in a cohort of 173 patients with HK; in addition, cross-resistance to GCV and FOS is determined.…”

“…Some cases of herpes simplex virus (HSV) keratitis refractory to topical acyclovir (ACV) treatment were previously reported as epithelial keratitis [1, 2]. Conventional diagnosis of an ACV-resistant HSV keratitis using a viral culture following in vitro drug sensitivity testing [1] is generally difficult and complicated.…”

“…Conventional diagnosis of an ACV-resistant HSV keratitis using a viral culture following in vitro drug sensitivity testing [1] is generally difficult and complicated. Recently, we reported a simple and useful method for diagnosing drug-resistant herpetic infection using real-time polymerase chain reaction (PCR) by monitoring the changes in the viral DNA copy numbers before and after ACV treatment [3].…”
